1) Grades within fuel station area, north of kiosk, shall be directed toward stormceptor.
2) Copy of stamped approved grading plan(s), permit(s), NOI, AzPDES general permit, geotechnical report/addenda, and other reports shall be kept at site at all times, until final grading approval.
3) Project shall conform to recommendations and requirements found on approved plans (Ali Fakih P.E. SEG) for purpose of grading construction of a Fry's and fuel center commercial building site at 1801 W Valencia Rd and associated other commercial pad grading and associated approved documents including Speedie and Associates soils/geotechnical engineering reports for project and any subsequent approved addenda (Lyle Tweet P.E.).
4) Project is located within a FEMA floodplain zone X-shaded as shown on FEMA FIRM number 040076 C 2270 L, which has been determined to be a jurisdictional 100-yr floodplain with flows less than 1-foot, and is also located within the WBSCR (West Branch Santa Cruz River) Diversion Channel Watershed TSMS (BG), a TSMS (Tucson Stormwater Management Study) Balanced Basin Management Area.
5) Call for Pre-construction meeting. For a PDSD Engineering Inspection, call IVR (791-3111), or schedule with a Customer Service Representative at Planning & Development Services Department (PDSD), or contact PDSD Engineering at 837-4888, or schedule inspections online at: http://cms3.tucsonaz.gov/pdsd/inspections
6) Some grading construction activities require separate special inspections by EOR or consultants. During these inspections PDSD Inspectors should be present. (Ex: During installation of stormceptor and underground detention/retention systems, bleed pipe from subsurface detention/retention system, or other stormdrain pipe installations). Discuss during Pre-con or contact PDSD Engineering Inspector to confirm which inspections may be needed.
7) See the associated building permit T15CM05730 and associated floodplain use permit T15OT01092. Separate Elevation Certificates shall be required for all buildings to be constructed at this commercial site. Separate Elevation Certificate under a separate Floodplain Use Permit will be required for Kiosk at Fuel Center; submit Floodplain Use Permit Application when ready to construct.
8) Pre-slab Elevation Certificate must be submitted PRIOR to pouring slab. Finished Construction Elevation Certificate is required before Final Grading Inspection Approval. See Floodplain Use Permit T15OT01092 for Fry's Elevation Certificate requirements.
9) Elevation Certificates for buildings will be required to show a minimum FFE of 1-ft above 100-year BFE and adjacent grades at or above the adjacent BFE. (Minimum FFE for Fuel Kiosk: 2475.50, and minimum FFE for Fry's: 2477.20.)
10) No trees shall be located within the SVT's. Field adjustment of new trees may be needed in SVT locations, and EOR shall assist contractor to assure placement of proposed trees and any other proposed plants are outside of SVT's. Any existing trees within SVT's shall be checked and trimmed to assure that they are clear of leaves and branches to a height of at least six feet above grade. Location of trees within existing and future sight visibility triangles may be restricted or modified as determined by EOR or City of Tucson Inspectors in order to preserve visibility.
11) Contractor is responsible to protect in place any existing utilities; all work within these easements must be coordinated with the proper utilities prior to any site construction. All recordation dkt/pg or sequence numbers shall be listed on subsequent plans for revisions or as-builts.
12) For inspection purposes, post City of Tucson's "AZPDES - Posting Requirements" green sheet, or the NOI, at construction entrance of site, at beginning of construction activities, and maintain this posted document throughout project construction.
13) Any remaining signatures from Operators must be on site copy of SWPPP (exhibit or report) at or before commencement of construction. All Operators shall be identified and have separate certification statements. Assure these certifications are available on-site.
14) Interim control measures shall be placed within approved project limits. After interim controls are removed, permanent erosion control are required on all disturbed areas, in form of vegetative cover, gravel, sized decorative rock, and/or other controls. Prior to final approval all temporary controls shall be removed and permanent controls shall be functioning in place.
15) Operator shall report to ADEQ any noncompliance (including spills) which may endanger human health or the environment. Operator shall orally notify office listed below within 24 hours: Arizona Department of Environmental Quality, 1110 W. Washington, 5th floor (5515B-1), Phoenix, AZ 85007 Office: 602-771-4466; Fax 602-771-4505
16) Receiving waters are West Branch Diversion Channel.
17) Stabilized entrances shall be maintained at all active access entrances. Use of water trucks or other dust controls shall be utilized, and construction entrances shall be maintained to minimize sedimentation, dust issues, and tracking.
18) TDOT improvements shall be completed as a part of the improvements prior to release of CofO. See PIA plans. Recordation of private drainage maintenance easement shall be shown on as-builts.
19) Any revisions to Grading Plan sheets must be reviewed by PDSD Engineering Engineer and may include discussion, details, or a revised grading plan. Call PDSD Engineering Division at 791-5550 (or Elizabeth Leibold P.E. 837-4934) to discuss any revisions.
20) If grading construction is expected to last longer than expiration date of permit, contact PDSD to renew/extend permit. If final grading inspection has not been completed before permit expires, and permit has not been renewed, additional fees and reviews may be required. Requests for permit extensions may be submitted to PDSD (attn: Claudia Buford), prior to permit expiration, for consideration of grading plan extension approval.
21) Permittee shall notify PDSD when grading operation is ready for final grading inspection. Final grading approval shall not be given until all work, including installation of all drainage improvements and their permanent protective devices, and all erosion control measures have been completed in accordance with approved grading plan and grading permit, and all conditions of permit are completed.
22) As-builts are required to be submitted. The as-built / "Final Revised Grading Plan" and substantial completion letter(s) may be submitted under activity number DP15-0126 to PDSD permit clerks. The documents will be reviewed and then if acceptable, a copy of the as-builts will be provided to PDSD Engineering Inspector for final grading inspections. For grading permit project closure documentation, submit: cover letter explaining which sheets were changed for the as-builts, one copy of original grading plans, 2 copies of sheets that show as-built information, letters of completion (one for drainage improvements, and another letter for substantial completion of overall project, and clarification of drainage maintenance responsibility). As-builts shall include: seal on sheets that show as-built data including FFE, elevations of constructed underground detention/retention system, stormceptor, inverts of pipes and inlets/outlets, fuel station area, cover for pipes, and general grades as determined by RLS to show substantial completion for onsite improvements of project. Also include sequence number of recorded private drainage maintenance easement for offsite drainage improvement on as-builts.
