Pursuant to UDC Sec. 1.5.1.B, you will need to provide a request letter with the document upload, which should outline: a. The name and address of the person requesting determination, presumably yourself b. The code or part therefore that requires determination or clarification, namely the use you believe Oxford House Sahuaro should be categorized as c. The requesting party’s proposed determination or clarification, specifically the justification for this based upon the City’s Unified Development Code d. Reference to any City pending applications or requests for approval or code enforcement actions e. Additionally, if would be particularly helpful to include information regarding what use Oxford House is characterized as by the State of Arizona that entitles them to State funding.
