Traffic Control Plan: 1. Please update Note C: “Sidewalk will remain open and pedestrian traffic will not be affected.” 2. Please include a sidewalk plan showing the work areas on both sides of the sidewalk. 3. Please direct bicycles onto the sidewalk where the bicycle / bus lane will be closed with a sign reading “BICYCLES ON SIDEWALK OK / YIELD TO PEDS”. E-mail me for the specs on this sign if you need them. 4. Show the length of the taper at the start of the closure (12ft x 40 mph x 40 mph / 60 = 320’) and at the end of the closure (100’) and include a buffer space of 305’ between the leading taper and the work area. 5. Remove the SHARE THE ROAD WITH CARE sign. 6. Please include an arrow board at the start of the lane closure taper. 7. Show all bus stops. If planning to temporarily move the bus stop, coordinate with SunTran dispatch 520-206-8934. Site Plan: 8. PUAM Section 9.2 – Please submit a Design Waiver Request. Utility permit applications should have a site map that shows all the offsets between ROW Limits and Easements, Existing Utilities, Back of Curb or Edge of Pavement, and Location of Sidewalk. However, because the proposed work is mostly aerial work on existing poles and because the sidewalks will not be affected, this application can be approved without the required site map if you also submit a Design Waiver Request. This is a short letter described in Section 9.5 of the Utility Manual: Some utility installations do not need the same level of detail than is typically required for a “New Project”. In those cases, a written request for waiver requesting simplification of the document may be submitted with the permit application justifying the omission of detail. Waiver requests are reviewed on a case-by-case basis and may not be applied to any future work. Please submit a Design Waiver Request as a supplemental document attached to this application.
