Site Plan:

1. On a demolition plan, show the curb, the existing sidewalk, the distance between the curb and the sidewalk and the width of the existing sidewalk. Also show which sections will be demolished by shading these sections and give dimensions to the sections planned for demolition.
2. Also submit a site plan with the same information showing the proposed repairs. If you plan to replace the sidewalk exactly as it originally was, the site plan will be same as the demolition plan.
3. Please also include the name and contact info of the contractor, property owner and plan designer.

4. Policy 6.01.1 - Instead of closing the bike lane, please provide a space for bikes behind the barricades. Use a second line of barricades to create a 5’ shifting bike lane. Email me with questions or for an example.
5. If closing sidewalk, include a sidewalk control plan (See MUTCD Figure 6H-28 for an example. Email me with questions.)
6. Pedestrian route must be safe, detectable, and accessible including ramps, truncated domes and a detectable edge with Longitudinal Channeling Devices (LCDs)
