Engineering review required prior to issuance, floodplain use permit TP-FPU-0526-00214 must have fees paid prior to issuance. 2) One of the following is required: a) A contractor with an active Arizona Contractor’s License for the applicable scope of work OR b) A completed and signed Owner-Builder Affidavit (OBA). The Contractor and/or Owner must be correctly identified on the permit, and each must have a TDC Online account in order to be added.
