Please see the link below to access the affidavit and resubmit to the permit. You may also add a contractor to the permit if desired. Prior to permit issuance, one of the following is required: 1) A contractor with an active Arizona Contractor’s License for the applicable scope of work, OR 2) A completed and signed Owner-Builder Affidavit (OBA). The Contractor and/or Owner must be correctly identified on the permit, and each must have a TDC Online account in order to be added as a contact.
