If Alfredo Prince is acting as the Legal Representative, please submit documentation verifying this. If Alfredo Prince is not the Legal Representative, please submit a new Owner/Builder Affidavit completed by Carmina Prince OR add a contractor to the permit that has a valid Arizona ROC license number. The Contractor and/or Owner must be correctly identified on the permit, and each must have a TDC Online account in order to be added.
