As the new entity occupying this space, you are seeking a New Certificate of Occupancy… correct? 2. STATE OPERATOR’S LICENSE a. Please do ensure to apply for your operator’s license with Arizona Department of Health Services. 3. SHEET TS1 a. Please correct your Minimum Required Plumbing Fixtures. i. Per IBC Table 2902.1, the required number of Water Closet is 3 (you show 2), please correct this. ii. Per your floor plan you show 4 water closets. Sheet TS1 shows 3. Please correct this. iii. Per IBC Table 2902.1, the required number of Lavatories is 3 (you show 2), please correct this. iv. Per your floor plan you show 4 lavatories. Sheet TS1 shows 3. Please correct this. b. Drinking Fountain and Service Sink i. Please locate and label the drinking fountain and service sink on your floor plan. 4. SHEET A1 a. Please comply with the lavatory requirement below. i. Lavatory shall be maximum 34 inches above floor to the higher of the rim or counter surface, faucets and soap dispenser shall have a reach depth of 11 inches per ANSI 606.5 ii. Metered faucets shall remain open for 10 seconds minimum per ANSI 606.4 iii. Water supply and drain pipes under lavatories shall be insulated to protect against contact per ANSI 606.6 b. RECEPTION COUNTER: Please provide drawings illustrating compliance to the following. i. The tops of work surfaces shall be 28 inches min & 34 inches max above the floor per ICC A117.1 sec 902.4. ii. Sec 306.3.3 Knee clearance shall be 11 inches min in depth at 9 inches above the floor, and 8 inches min in depth at 27 inches above the floor. iii. Sec 306.2.4 Additional toe clearance space extending greater than 6 inches beyond the available knee clearance at 9 inches above the floor. iv. Sec 306.3 Knee clearance shall be 30 inches min in width. v. 904.3.2 Parallel approach, the counter surface 36 inches min length. vi. 904.3.3 Forward approach, the counter surface 30 inches min length. 5. SHEET A3: ROOM FINISH SCHEDULE – walls with GWB/painted a. All Restrooms must comply to the following. Please make the necessary corrections. i. Walls and partitions within 2 feet of service sinks, urinals & water closets shall have a smooth, hard, nonabsorbent surface, to a height of not less than 4 feet above the floor, and except for structural elements, the materials used in such walls shall be of a type that is not adversely affected by moisture.
