COMMENTS:
1. Two suites are being combined into one of which suite 102 is being used only.
a. A new certificate of occupancy shall be generated to reflect this condition. Please provide all information required to complete the certificate of occupancy form correctly.
b. Complete the Code Analysis to include the following:
i. Provide the description of the activity that will be operating in this space. Is it doctor’s offices, is it an outpatient facility or is a behavioral health facility, etc.
ii. Provide the occupant load calculation.
iii. Provide the minimum plumbing fixture calculation, what is required and what is provided.
iv. On the floor plan, locate and label the drinking fountain(s), service sink & restrooms.
c. Provide the floor plan for the Means of Egress. The combining of the two suites may have changed the pattern.
i. Ensure to show the length of travel.
ii. At the point of exit, enumerate the exit width required and exit width provided and the number of occupants exiting at the point of exit.
