NOTE: R-4 maximum allowable is 10 clients (indicate the number of clients on your drawings). 1. On the Site Plan show/indicate the Accessible route from drop off location to the entry door complying to ANSI 117A. In an Emergency, show/locate the gathering area where it is safe from danger. 2. Indicate the clear opening of the doors complying to ANSI 117A. 3. Not used 4. Show/locate the two grab bars at the Water Closet as well as the two grab bars at the Shower, make sure to indicate the installation of Structural Backer Board for these grab bars. 5. Provide Emergency Escape & Rescue Openings per IRC R310 at every bedroom/sleeping space. Adjacent to the opening(s), indicate the clear opening for both horizontally and vertically and provide the dimension from the finish floor to the bottom of the opening. Indicate if there are obstacles on either side of the opening. 6. Fire Sprinkler is required and processed as a separate permit. Submit the drawings to the Fire Department for review and approval. They will also conduct inspection, their approval will be required before issuing the Certificate of Occupancy. 7. Electrical work related to installation of Emergency Lights, Lighted Signs, Audible Fire Alarm shall also be reviewed as part of this permit by submitting Electrical drawings. An inspection will be conducted by our inspectors prior to issuance of the Certificate of Occupancy. 8. Any new work must be included as part of this building permit. 9. An Operator's License must be applied for with the State, the absence of this license shall void this building permit. 10. Submit a copy of the State License application as part of this permit. 11. Submit a Narration as to how facility handles any persons in this facility receiving custodial care who require limited verbal or physical assistance responding to an emergency situation to complete building evacuation and self preservation. 12. Provide a Residential Care Services land use type as defined by UDC Section 11.3.7.D with your next submittal. “Group home” is not a land use defined within the UDC. Additionally, show the location of the four required motor vehicle parking spaces as required by UDC Table 7.4.4-1. 2. Show all structures on the site plan. A detached accessory structure in the southwest corner of the property appears in aerial photography beginning in 2018. The structure does not appear to meet perimeter yard setbacks as required by UDC Table 6.3-2.A. The structure may remain if it can be demonstrated that the structure is less than 200 square feet and is not connected to any utilities.
