Starting at the TEP pole in front of 7475 E 22nd St begin overlash of 144ct fiber heading north for 277', turn west and continue overlash for 1334' then turn northwest for 54' continue north for 334' and then turn west for 497' and riser down and begin trenching for 16' heading east and then turn south and trench 14' across the alley and on to private property for 59'. PROOF OF NOTIFICATION IS REQUIRED AT THE TIME OF THE PRECONSTRUCTION MEETING. PAVEMENT PATCH TYPE SHALL BE IN CONFORMANCE TO THE CURRENT EDITION OF THE CITY OF TUCSON PUBLIC UTILITY ADMINISTRATIVE MANUAL. BUS STOP IN THE AREA, CONTACT SUN TRAN DISPATCH, AT 520-206-8934 AND/OR BEA PAULUS AT 520-206-8826 AT LEAST 48-BUSINESS HOURS PRIOR TO START OF WORK FOR COORDINATION. Alley must be restored to existing or better condition. Alley surface must be graded and compacted to City Standards and Specifications for Public Improvements and existing drainage patterns must be restored and maintained. Any disturbed gravel must be replaced.

| 02/11/2022 | February 7, 2022 Floodplain Use Permit: TDOT1688 The applicant is authorized to use a portion of the flood hazard area Rose Hill Wash at 7139 E 22nd Street, for the purpose of installing fiber optic cable (Arial). The applicant agrees to comply with City of Tucson Floodplain and Erosion Hazard regulations and is particularly bound by the conditions stated below. Violation of any of the conditions stated or referred to below, as determined by the City Engineer, shall be grounds for revocation of this permit. Responsibility for renewal of this permit, if necessary, is with the applicant. Issuance of this permit does not constitute a waiver of other requirements contained in the Tucson City Code, Unified Development Code, or any other Federal, State, or local regulation. No use other than as permitted herein is allowed. Additional uses allowed under City of Tucson Floodplain and Erosion Hazard Regulations and Watercourse Amenities, Safety, and Habitat regulations require the issuance of an additional Floodplain Use Permit. The following conditions shall apply to this permit. 1) This permit becomes invalid and is subject to additional floodplain review, if any required development changes the construction of the building or site improvements, or, if there is any further disturbance within the floodplain other than that outlined in this application. This floodplain use permit may be subject to conditions or restrictions designed to reduce or mitigate the potential damage or hazard to life or property resulting from new building and site improvements within the regulatory floodplain, floodway or erosion hazard areas as determined by the project engineer and accepted by the City of Tucson. 2) All work shall conform to the attached plans T22RW00313 which are made part of this permit. 3) Except as approved by the City Engineer prior to the start of construction, development under this permit shall not obstruct, retard, or divert the existing flow of floodwaters from or to adjacent properties. 4) At finished construction, notify DTM for final inspection to assure watercourse does not have any construction material in or around watercourse, and any construction debris has been removed from the right-of-way and floodplain and restored to natural grade, if disturbed. 5) Any additional future disturbance of the floodplain shall have separate floodplain use permit applications submitted to this office for review and approval. This Floodplain Use Permit will be automatically revoked and becomes invalid if conditions are not complied with, as set forth in this permit. |
