EP NEEDS TO ADD ONE POLE ON THE EAST SIDE OF HERBERT BEHIND ADDRESS 812 S 4TH AVE AT SOUTH SIDE OF PROPERTY TO ACCOMODATE INCREASE IN SERVICE. WILL ALSO NEED TO CHANGE OUT EXISTING POLE AT NORTH SIDE OF PROPERTY ON EAST SIDE OF HERBERT FOR MAINTENANCE AND ADD ANCHOR. WILL NEED TO ADD ANCHOR AT POLE LOCATED EAST SIDE OF HERBERT NEAR ADDRESS 824 S 4TH AVE TO ACCOMODATE CONDUCTOR SIZE INCREASE. TOTAL OF 4 HOLES ON EAST SIDE OF HERBERT IN ROW. WILLL NEED TO CHANGE OUT POLE IN YARD OF 812 S 4TH AVE AS WELL AND THIS WILL REQUIRE CRANE TO SET FROM 4TH AVE. WILL ALSO NEED TO CHANGE OUT POLE IN YARD OF 812 S HERBERT AND THIS WILL POSSIBLY NEED TO BE REACHED FROM HERBERT WIHT CRANE AS WELL
NOTE: This project is located in an Archaeological Sensitivity Zone. An Archaeological Monitor hired by the permit applicant is required to be on-site during any digging, unless a waiver is issued.
