Starting on the S/W corner of E. 12th St and S. Herbert Ave at existing vault pull fiber N 25' through existing conduit, riser up pole, Overlash .144 ct fiber going North 70 ', East 470 ' , South 58 ', East 220 ' , North 60' Continuing East 50' riser down pole and trench / bore 15' South, 35' East, 15' North placing a 3x3x3 pothole for new Vault, from vault tie in to existing Verizon vault. NOTE: E 12TH ST is under a pavement moratorium, any new asphalt that is disturbed will require a 2” mill and overlay for 50’ in each direction from the outside of the cut for all effected travel lanes. This project is located in an Archaeological Sensitivity Zone. An Archaeological Monitor hired by the permit applicant is required to be on-site during any digging, unless a waiver is issued.
