PLEASE SEE ATTACHED SITE PLANS FOR MORE DETAILS AS WELL AS TC PROK MAY MED DURING THE EVENING HOURS AS WELL. ADDRESS ON APPLICATION-4390 E GRANT RD 09/19/2019 PLAN CHANGE B&F will be adding an additional Excavation repair of 8'' Sewer. Outside of the Collar work called out on the Existing Permit. The address for the excavation is 4390 E Grant Rd. and is located 122LF from manhole 4981-30 and will be a 10 lf repair centered from that point see map
