10/27/2016 FIRE INSP - STATE LICENSE nsprenger Approved TFC 703.1 Maintenance. The required fire-resistance rating of fire-resistance-rated construction (including walls, fire-stops, shaft enclosures, partitions, smoke barriers, floors, fire-resistive coatings and sprayed fire-resistant materials applied to structural members and fire-resistant joint systems) shall be maintained.
1) You shall repair/replace fire resistant rating through the fire wall that is above the ceiling tiles and between the clinic and the new construction (East wing).
2) Room #141 (storage room) There is 'orange conduit' under the stairs and penetrating a fire wall. This is an area required to be protected by a 1 hour wall and all penetrations shall be sealed with an approved fire retardant material of equal fire resistant rating.
3) Where penetrations for computer cable have been made through 1 hour rated fire walls (above and below ceiling tiles), the openings shall be protected with a fire rated foam of equal fire resistance rating.

TFC 703.2 Opening protectives. 'Opening protectives' shall be maintained in an operative condition in accordance with NFPA 80. Where allowed by the fire code official, the application of field-applied labels associated with the maintenance
of 'opening protectives' shall follow the requirements of the approved third-party certification organization accredited for listing the opening protective. Fire doors and smoke barrier doors shall not be blocked or obstructed, or otherwise made
inoperable. Fusible links shall be replaced promptly whenever fused or damaged. Fire door assemblies shall not be modified.
1) Janitor room #150 on 1st level (fire door was altered by placement of vents after-market). You shall replace the door with an approved, non-modified fire door with the same required rating.
2) The 'hold open' foot throw device on the first floor door to the lobby, and all (office, conference, break room, storage, bathroom, exit/entrance, and other) doors opening directly to corridors, SHALL BE REMOVED. This is




not an acceptable door holder for a fire door per NFPA 80. These corridors are fire rated.

TFC 604.5 Emergency lighting equipment. Emergency lighting shall be inspected and tested in accordance with Sections 604.5.1 through 604.5.2.1.

MONTHLY TEST
TFC 604.5.1 Activation test. An activation test of the emergency lighting equipment shall be completed monthly. The activation test shall ensure the emergency lighting activates automatically upon normal electrical disconnect and stays sufficiently illuminated for a minimum of 30 seconds.
TFC 604.5.1.1 Activation test record. Records shall be maintained on the premises for a minimum of three years and submitted to the fire code official upon request. The record shall include the location of the emergency lighting tested, whether the unit passed or failed, the date of the test, and the person
completing the test.
1) You shall perform a monthly test of every emergency light that is on the property, including all exterior emergency lighting, and document the location of each, 'pass or fail', 'test date', and 'person completing test'. This record shall be maintained and presented to the fire inspector upon request at the next inspection of your facility.

YEARLY TEST
TFC 604.5.2 Power test. For battery-powered emergency lighting, A power test of the emergency lighting equipment shall be completed annually. The power test shall operate the emergency lighting for a minimum of 90 minutes and shall remain sufficiently illuminated for the duration of the test.
TFC 604.5.2.1 Power test record. Records shall be maintained on the premises for a minimum of three years and submitted to the fire code official upon request. The record shall include the location of the emergency lighting tested, whether the unit passed or failed, the date of the test, and the person completing
the test.
1) You shall perform a yearly test of every emergency light that is on the property, including all exterior emergency lighting, and document the location of each, 'pass or fail', 'test date', and 'person completing test'. This record shall be maintained and presented to the fire inspector upon request at the next inspection of your facility.








TFC 605.6 Unapproved conditions. Open junction boxes and open-wiring splices shall be prohibited. Approved covers shall be provided for all switch and electrical outlet boxes.
1) You shall repair/replace the broken switch plate that is in the storage closet under the stairs and in the 'Visitation Area' Room #224.

TFC 605.5 Extension cords. Extension cords and flexible cords shall not be a substitute for permanent wiring. Extension cords and flexible cords shall not be affixed to structures, extended through walls, ceilings or floors, or under doors or
floor coverings, nor shall such cords be subject to environmental damage or physical impact. Extension cords shall be used only with portable appliances.
1) You shall remove the extension cord from the power strip in Room # 211 that is powering the Fax/printer.
2) You shall remove extension cord powering the wall mounted fan in room # 233.

TFC 605.4.2 Power supply. Re-locatable power taps shall be directly connected to a permanently installed receptacle.
1) The power strips between Elizabeth's and Sasha's desks (in the business office) shall each be connected directly to an outlet and not to each other.
