12/18/2015 December 15, 2015 FLOODPLAIN USE PERMIT # T15OT01092 PROJECT ADDRESS: 1801 W Valencia Rd, Tucson AZ 85746 APPLICANT'S NAME: FRY'S DIVISION OFFICE PROPERTY OWNER: TITLE SECURITY This parcel lies in a FEMA zone X-shaded which is determined to be a jurisdictional 100-yr floodplain with flows less than 1-foot, within the WBSCR Diversion Channel Watershed (BG), FEMA map number 040076 C 2270 L. The applicant is authorized to use a portion of the flood hazard area in the City of Tucson, Arizona, on the property described as: Parcel 138-24-4030, 1801 W Valencia Rd SHOPS AT MIDVALE LOT 4, in a portion of Township 15 South, Range 13 East, Section 15, for the purpose of constructing Fry's commercial building. The applicant agrees to comply with City of Tucson Floodplain and Erosion Hazard, and Watercourse Amenities, Safety and Habitat regulations, and are particularly bound by the conditions stated below. Violation of any of the conditions stated or referred to below, as determined by the City Engineer, shall be grounds for revocation of this permit. Responsibility for renewal of this permit, if necessary, is with the applicant. Issuance of this permit does not constitute a waiver of other requirements contained in the Tucson City Code, Land Use Code, or any other Federal, State, County, or local regulation. No use other than as permitted herein is allowed. Additional uses allowed under City of Tucson Floodplain and Erosion Hazard Regulations and Watercourse Amenities, Safety, and Habitat regulations require the issuance of an additional Floodplain Use Permit. The following 9 conditions apply to permit T15OT01092: 1) Call for Pre-construction meeting, elevation certification inspections, and final inspections. For a PDSD Engineering Inspection, call IVR (791-3111), or contact PDSD Engineering at 837-4991, or schedule inspections online at: http://www.ci.tucson.az.us/dsd/Online_Services/Online_Permits/online_permits.html 2) This Floodplain Use Permit shall be kept in a readily accessible location on site at all times. 3) Within 5 (five) days of issuance of this permit, and through completion of the project, notification must be posted and maintained in a conspicuous place on the job site stating "CONSTRUCTION PROCEEDING UNDER REQUIREMENTS OF CITY OF TUCSON FLOODPLAIN USE PERMIT # T15OT01092. FOR INFORMATION PLEASE CALL 791-5550." 4) Submit Elevation certification(s) by a Land Surveyor registered in Arizona for the pre-slab forms as well as the finished construction finished floor elevation for the building to be installed. Minimum finished floor elevation of Frys's building shall be set at 2477.20 NAVD88 based on approved grading plan. PDSD Engineering has the preliminary elevation certificate filled out for the RLS to use as needed. Assure the lowest elevation of the lowest service equipment is provided in C2e) and E4. Notations should be added to clarify type of equipment and location. 5) Pre-slab Elevation Certificate must be submitted PRIOR to pouring slab. Finished Construction Elevation Certificate is required before Final Grading Inspection Approval. 6) Except as approved by the City Engineer prior to the start of construction, development under this permit shall not obstruct, retard or divert the existing flow of floodwaters from or to adjacent properties. 7) This permit becomes invalid and is subject to additional floodplain review, if any required development permit changes the configuration of the improvement, or, if there is any further disturbance within the floodplain other than that outlined in this application. This floodplain use permit may be subject to conditions or restrictions designed to reduce or mitigate the potential damage or hazard to life or property resulting from development within the regulatory floodplain, floodway or erosion hazard areas as determined by the project engineer and accepted by the City of Tucson. 8) Separate Elevation Certificates shall be required for all buildings to be constructed at this commercial site. Separate Elevation Certificate under a separate Floodplain Use Permit will be required for Kiosk at Fuel Center; submit Floodplain Use Permit Application when ready to construct. 9) See the associated building permit T15CM05730 and associated grading permit DP15-0126. This Floodplain Use Permit will be automatically revoked and becomes invalid for non-compliance to conditions set forth in this permit. Also, this permit will expire one year from issue date. Renewal of this permit will require new application, review and approval of application by City of Tucson Planning & Development Services Dept, Engineering Division.
