02/23/2015 | The following 19 conditions apply to permit T14OT01389: 1) Call for Pre-construction meeting and final inspections for #91(FF) and #97(EZ). For a PDSD Engineering Inspection, call IVR (740-6970), or contact PDSD Engineering at 837-4991, or call Elizabeth Leibold P.E., CPM, CFM at (520) 837-4934, or to schedule the inspections online go to: http://www.ci.tucson.az.us/dsd/Online_Services/Online_Permits/online_permits.html 2) Each pole shall be constructed to meet specific design depths per approved plans with depth of subsurface foundations meeting requirements for scour depths. 3) This Floodplain Use Permit & plans shall be kept in a readily accessible location at site during scheduled inspections for TEP structures #91(FF) and #97(EZ). Contractor shall have copy of permit at all times. 4) All work shall conform to the approved plans prepared by TEP, Scour Statement (2nd submittal) and associated addenda prepared by EEC (Bruce Wilson P.E.), and associated Geotechnical Engineering Reports and addenda. All construction activities shall be located within an existing electrical easement or TEP/Unisource parcel. 5) Structure #97(EZ) shall be set no closer than 55-ft from perpendicular line from top of existing bank protection. 6) Geotechnical Engineer shall be present during excavation activities for foundation and pour for structures #91(FF). 7) All power pole material strength criteria and construction shall comply with any applicable Federal Technical Bulletins, to assure flood damage-resistant materials are used for the foundation/pole, and, to prevent collapse from hydrodynamic loading during 100-year storm event. 8) All power pole construction activity and installations shall be completed prior to June 1, 2015. 9) Install temporary construction fencing between proposed pole locations and bank protection sub-embankment when the proposed pole is less than 50-ft in distance from top of bank protection or as recommended by civil or geotechnical engineer. Verify distances for poles #91(FF) and #97(EZ). 10) Geotechnical Engineer and the Civil Engineer for pole #91(FF) shall be present during appropriate construction phases of the project to determine the proper construction steps are being followed. This shall include having a Geotechnical Engineer present for observations of soils encountered for foundation installation. 11) Free-fall concrete is not accepted for construction method for these pole foundations under this permit. For poles #91(FF) and #97(EZ), bottom-dump hopper, or elephant trunk discharging concrete near hole bottom, shall be used, or as determined by Geotechnical Engineer. 12) Compaction testing is required before each pole construction begins and after each pole installation is completed to show that the compaction surrounding poles #91(FF) and #97(EZ) are at, or exceeds, the existing compaction, and, is compacted to a density of not less than 95 percent of the maximum density and per PC/COT Standard Specifications (2003) section 203-9.03(B). 13) All unused materials, old lattice structure, and construction equipment must be removed from each construction site prior to final approval. 14) All disturbances due to the construction of this project within existing banks in the Santa Cruz River shall be mitigated to pre-construction condition to include: re-grading disturbed soils to pre-construction grades. Construction access or other activity shall not damage any existing bank protection. 15) PDSD Engineering Inspections are required for poles #91(FF) and #97(EZ) prior to final floodplain inspection. 16) Inspection field reports are required and must be submitted to the City of Tucson Planning & Development Services Department (PDSD) Engineering Division. City Inspector may request the reports during inspections, however a copy must be provided with as-built submittal. 17) Submit final revised plan (as-built) to PDSD under the activity T14OT01389, prior to final inspection. Project closure documentation shall include as-built plans showing elevations of top of concrete pad and adjacent finish elevations, testing reports from Geotechnical Engineer, and final completion statement from Civil Engineer. 18) Except as approved by the City Engineer prior to the start of construction, development under this permit shall not obstruct, retard or divert the existing flow of floodwaters from or to adjacent properties. 19) This permit becomes invalid and is subject to additional floodplain review, if any required development permit changes the configuration of the improvement, or, if there is any further disturbance within the floodplain other than that outlined in this application. This floodplain use permit may be subject to conditions or restrictions designed to reduce or mitigate the potential damage or hazard to life or property resulting from development within the regulatory floodplain, floodway or erosion hazard areas as determined by the project engineer and accepted by the City of Tucson. |
