07/01/2013 | FLOODPLAIN USE PERMIT: T13OT00747 (Generator Equipment Yard) PROJECT ADDRESS: 1601 W Saint Marys Rd, Tucson Arizona 85745 PROPERTY OWNER: St Marys Hospital This project lies in a FEMA Zone A (map number 04019C 2260L). The applicant is authorized to use a portion of the flood hazard area in the City of Tucson, Arizona, on the property described as: The Generator Equipment Yard located at the northwest corner of the parking structure at 1601 W Saint Marys Rd, also known as a portion of PTN FRAC LOTS 1 2 10 11 & 12 N2 SE4 LYG W OF SILVERBELL & S OF ST MARY'S & N OF ANKLAM 17.17 AC, Tucson Arizona, within Township 14S, Range 13E, Section 10, Parcel 116-15-088L, Lat 32.2271, Long -111.0013, for the purpose of construction of a pad for generator equipment within the FEMA A zone within the Silvercroft Wash Watershed breakout floodplain from the non-certified berm. The applicant agrees to comply with City of Tucson Floodplain and Erosion Hazard, and if applicable, Watercourse Amenities, Safety and Habitat regulations, and are particularly bound by the conditions stated below. Violation of any of the conditions stated or referred to below, as determined by the City Engineer, shall be grounds for revocation of this permit. Responsibility for renewal of this permit, if necessary, is with the applicant. Issuance of this permit does not constitute a waiver of other requirements contained in the Tucson City Code, Land Use Code, or any other Federal, State, County, or local regulation. No use other than as permitted herein is allowed. Additional uses allowed under the City of Tucson Floodplain and Erosion Hazard Regulations require the issuance of an additional Floodplain Use Permit. The following 11 conditions apply to permit T13OT00747: 1) This project is located in Zone A as shown on upcoming FIRM panel 2260L. Although a second CLOMR has been issued for future construction of a certified levee, these proposed improvements may be impacted by the FEMA floodplain in the mean time. 2) This Floodplain Use Permit T12OT00747 is issued concurrently with the associated improvement plans/permit. Floodplain Use Permit & plans shall be kept in readily accessible location on site at all times. 3) Within 5 (five) days of issuance of this permit, and continuing through to the completion of the project, notification must be posted and maintained in a conspicuous place on the job site stating "Construction proceeding under requirements of City of Tucson Floodplain Use Permit No. T13OT00747. For information please call 791-5550." 4) PDSD Engineering Inspections are required prior to final floodplain inspection. The applicant attests to the fact that the plans and documents submitted represent all of the improvements that will be done are included in the construction plans and/or documents herewith. No other improvements, repairs, reconstruction, additions or remodeling have been made to structures that is not indicated in the attached plans and/or documents. This permit is specific to the improvements for the building specified as indicated on the approved plans. It shall not apply to any other structure or property, whether attached or unattached, not specified under this permit. 6) Except as approved by the City Engineer prior to the start of construction, development under this permit shall not obstruct, retard or divert the existing flow of floodwaters from or to adjacent properties. The existing drainage patterns at the property lines shall not be obstructed nor changed. 7) This permit becomes invalid and is subject to additional floodplain review, if any required development permit changes the configuration of the improvement, or, if there is any further disturbance within the floodplain other than that outlined in this application. This floodplain use permit may be subject to conditions or restrictions designed to reduce or mitigate the potential damage or hazard to life or property resulting from development within the regulatory floodplain, floodway or erosion hazard areas as determined by the project engineer and accepted by the City of Tucson and PCRFCD. 8) No disturbance is permitted other than area as shown on the approved plans. 9) The minimum FFE elevation of utility pad = 2352.65 NAVD88. 10) Submit Elevation Certifications by a Land Surveyor registered in the State of Arizona for pad elevation for finished construction. PDSD Engineering has the preliminary elevation certificate filled out for the RLS to use, as needed. An El Cert is needed for Finished-Construction assuring lowest elevation of the utility equipment to service the building is provided in C2e) and comment section D on finished construction el cert. Notations should be added to clarify type of equipment and location. Submit color photos for Item A6 of the Elevation Certificates. Approved Final El Cert is required prior to final approval of permits for this development; assure the El Cert is provided to the City of Tucson PDSD Engineering Division. 11) This permit expires June 28, 2014. If extension is not requested prior to expiration date, renewal of this permit will require a new application, review and approval of that application by the City of Tucson, Planning & Development Services Department, Engineering Division. This permit may be renewed for up to one year, at the sole discretion of the City Engineer or designated representatives. 12) If any walls are to be built, separate permits may be needed. This Floodplain Use Permit will be automatically revoked and becomes invalid for non-compliance to conditions set forth in this permit. Also, this permit will expire 2 years from its date of issue. Renewal of this permit will require a new application, review and approval of that application by the City of Tucson, Planning & Development Services Department, Engineering Division.
