| 01/13/2011 | FLOODPLAIN USE PERMIT # T10OT02668 PROJECT ADDRESS: 1262 W Valencia APPLICANT'S NAME: TEP PROPERTY OWNER: City of Tucson This parcel lies in a FEMA mapped Zone X-unshaded (map number 040076 C 2238K). The applicant is authorized to use a portion of the erosion hazard area in the City of Tucson, Arizona, on the property described as: within Parcel 138-01-004D, at position of existing lattice structure located in Santa Cruz River, at approximately 32.145 degrees latitude, -110.994 degrees longitude, in a portion of the W2 W2 114.55 AC, in a portion of Township 15 South, Range 13 East, Section 11, for the purpose of replacing a TEP electrical power transmission line lattice structure with a single pole with 65-ft deep pier foundation within the erosion hazard area of the Santa Cruz River. The applicant agrees to comply with City of Tucson Floodplain and Erosion Hazard, and Watercourse Amenities, Safety and Habitat regulations, and are particularly bound by the conditions stated below. Violation of any of the conditions stated or referred to below, as determined by the City Engineer, shall be grounds for revocation of this permit. Responsibility for renewal of this permit, if necessary, is with the applicant. Issuance of this permit does not constitute a waiver of other requirements contained in the Tucson City Code, Land Use Code, or any other Federal, State, County, or local regulation. No use other than as permitted herein is allowed. Additional uses allowed under City of Tucson Floodplain and Erosion Hazard Regulations and Watercourse Amenities, Safety, and Habitat regulations require the issuance of an additional Floodplain Use Permit. The following 14 conditions apply to permit T10OT02668: 1) Call for Pre-construction meeting and final inspections. For a PDSD Engineering Inspection, call IVR (740-6970), or contact PDSD Engineering at 837-4991, or schedule inspections online at: http://www.ci.tucson.az.us/dsd/Online_Services/Online_Permits/online_permits.html 2) This Floodplain Use Permit and plans shall be kept in a readily accessible location on site at all times. Within 5 (five) days of issuance of this permit, and through completion of the project, notification must be posted and maintained in a conspicuous place on the job site stating "CONSTRUCTION PROCEEDING UNDER REQUIREMENTS OF CITY OF TUCSON FLOODPLAIN USE PERMIT # T10OT02668. FOR INFORMATION PLEASE CALL 791-5550." 3) All work shall conform to the approved plans prepared by TEP, Drainage Statement prepared by EEC (Bruce Wilson P.E. sealed 12/31/2010), and Geotechnical Report prepared by Terracon (Oleg Lysyj sealed 12-10-10). 4) Install temporary construction fencing between proposed pole location and sub-embankment to southeast of the proposed pole at a distance as recommended by civil or geotechnical engineer. 5) Geotechnical Engineer and the Civil Engineer for the project shall be present during appropriate construction phases of the project to determine the proper construction steps are being followed. This shall include having a Geotechnical Engineer present for observations of soils encountered for the drilled shaft installation. 6) Free-fall concrete is not accepted for this project due to depth of pier. Bottom-dump hopper, or elephant trunk discharging concrete near hole bottom, shall be used. 7) Compaction testing is required before project begins and after pole installation is completed to show that the compaction surrounding pole is at, or exceeds, the existing compaction, and, is compacted to a density of not less than 95 percent of the maximum density and per PC/COT Standard Specifications (2003) section 203-9.03(B). 8) All unused materials, old lattice structure, and construction equipment must be removed from the parcel prior to final approval. 9) All disturbances due to the construction of this project within existing banks in the Santa Cruz River shall be mitigated to pre-construction condition to include: re-grading disturbed soils to pre-construction grades. Construction access or other activity shall not damage any existing bank protection. 10) PDSD Engineering Inspections are required prior to final floodplain inspection. 11) Inspection field reports are required and must be submitted to the City of Tucson Planning & Development Services Department (PDSD) Engineering Division. City Inspector may request the reports during inspections, however a copy must be provided with as-built submittal. 12) Submit final revised plan (as-built) to PDSD under the activity T10OT02668, prior to final inspection. Project closure documentation shall include as-built plans with elevations of top of concrete pad and adjacent finish elevations, testing reports from Geotechnical Engineer, and final completion statement from Civil Engineer. 13) Except as approved by the City Engineer prior to the start of construction, development under this permit shall not obstruct, retard or divert the existing flow of floodwaters from or to adjacent properties. 14) This permit becomes invalid and is subject to additional floodplain review, if any required development permit changes the configuration of the improvement, or, if there is any further disturbance within the floodplain other than that outlined in this application. This floodplain use permit may be subject to conditions or restrictions designed to reduce or mitigate the potential damage or hazard to life or property resulting from development within the regulatory floodplain, floodway or erosion hazard areas as determined by the project engineer and accepted by the City of Tucson. This Floodplain Use Permit will be automatically revoked and becomes invalid for non-compliance to conditions set forth in this permit. Also, this permit will expire one year from its date of issue. Renewal of this permit will require a new application, review and approval of that application by the City of Tucson, Planning & Development Services Department, Engineering Division.
