01/14/2010 | January 29, 2010 FLOODPLAIN USE PERMIT # T10OT00107 PROJECT ADDRESS: 5301 E Grant Rd, Tucson, AZ 85712 PROJECT NAME: TMC Phase 1 APPLICANT'S NAME: Richard Prevallet, TMC PROPERTY OWNER: TMC The applicant is authorized to use a portion of the flood hazard area within a City of Tucson jurisdictional floodplain area owned by Pima County Arizona, on the property described as: Portion of Parcel 110-12-094A, within TUCSON MEDICAL CENTER LOT 5 EXC WLY PTN, for the purpose of improvements for the Phase 1 improvements for TMC. The applicant agrees to comply with City of Tucson Floodplain and Erosion Hazard, and Watercourse Amenities, Safety and Habitat regulations, and are particularly bound by the conditions stated below. Violation of any of the conditions stated or referred to below, as determined by the City Engineer, shall be grounds for revocation of this permit. Responsibility for renewal of this permit, if necessary, is with the applicant. Issuance of this permit does not necessarily constitute a waiver of other requirements contained in the Tucson City Code, Land Use Code, or any other Federal, State, County, or local regulation. No use other than as permitted herein is allowed. Additional uses allowed under City of Tucson Floodplain and Erosion Hazard Regulations and Watercourse Amenities, Safety, and Habitat regulations require the issuance of an additional Floodplain Use Permit. The following 13 conditions apply to this permit T10OT00107: 1) This permit is for TMC Phase 1 improvements that are within parcel 110-12-094A only. A separate permit will be required for the improvements on parcel 110-12-0980. 2) Please call Planning & Development Services Department 740-6970 (IVR) for final floodplain inspection. Floodplain Use permit is T10OT00107 (note the 4th digit is the letter "O", and the rest are zeros). 3) This permit becomes invalid and is subject to additional floodplain review, if any required development permit changes the configuration of the structure and / or site development, or, if there is any further disturbance within the floodplain other than that outlined in this application. This floodplain use permit may be subject to conditions or restrictions designed to reduce or mitigate the potential damage or hazard to life or property resulting from development within the regulatory floodplain, floodway or erosion hazard areas as determined by the project engineer and accepted by the City of Tucson. 4) Except as approved by the City Engineer prior to the start of construction, development under this permit shall not obstruct, retard or divert the existing flow of floodwaters from or to adjacent properties. There shall be no disturbance or stockpiling outside of the disturbance limits. 5) A portion of the improvements lies within FEMA SFHA Zone A. All work shall conform to all recommendations as set forth in the referenced report: TMC Phase 1 Drainage Report (RBF Engineers) and approved plans. 6) Within 5 (five) days of issuance of this permit, through completion of the project, notification must be posted and maintained in a conspicuous place on the job site stating "CONSTRUCTION PROCEEDING UNDER REQUIREMENTS OF CITY OF TUCSON FLOODPLAIN USE PERMIT # T10OT00107. FOR INFORMATION PLEASE CALL 837-4888." 7) Prior to commencement of any earthwork activities, it is recommended that Stormwater Pollution Prevention measures be placed along downstream edge of disturbance area to minimize sediment runoff. See also SWPPP as a part of Grading Permit T09BU01354. 8) A minimum of 18-inches setback is required at lip of existing Alamo Wash bank protection, with plantings located between the irrigation line and the lip of the bank protection. Positive grades for drainage must be provided along top of bank to minimize ponding behind lip of bank protection and provide positive drainage. 9) Receiving waters are Alamo Wash and Arcadia Wash. 10) Provide heavy duty construction fencing at approved disturbance limits along Wash areas. 11) Stop Work Order may be issued if disturbance occurs outside of approved limits as shown on the approved plans. There shall be no disturbance of Alamo or Arcadia Washes, unless for limited stormwater purposes and as discussed with the PDSD Inspector. Any disturbance of Alamo or Arcadia Washes with out authorization may result in Stop Work Order. 12) The water harvesting areas shall be cleaned out by removal of existing top layer of sediments prior to final approval. 13) Call PDSD Engineering for special Inspections for underground stormdrain system, Pediatric building courtyard drain pipes and manholes; pedestrian and access bridges, and other drainage improvement areas as determined in pre-con meetings or in the field by the Inspector. This Floodplain Use Permit will be automatically revoked and becomes invalid for non-compliance to conditions set forth in this permit. Also, this permit will expire two years from its date of issue. Renewal of this permit will require a new application, review and approval of that application by the City of Tucson, Planning & Development Services Department, Engineering Division.
