PROPERTY OWNER: Carondelet Health Network The applicant is authorized to use a portion of the City of Tucson Special Flood Hazard Area, specifically described as: PTN FRAC LOTS 1 2 10 11 & 12 N2 SE4 LYG W OF SILVERBELL & S OF ST MARY'S & N OF ANKLAM 17.17 AC SEC 10-14-13 For the purpose of installing x-ray equipment in the Emergency Department (ED), as proposed on the attached plans and documents for St. Mary's Hospital - Carondelet Health Network X-RAY #1 (1418) X-RAY #4 (1412), and X-RAY #5 (1410) prepared/sealed by Swaim Associates LTD. Architects AIA on 2/04. Should cumulative improvements to the existing structure equal or exceed 50% of its base value as determined by the Development Services Department, no other improvements will be permitted unless the structure is brought into compliance with the City of Tucson Floodplain and Erosion Hazard Area Regulations - Chapter 26, Tucson Code. The applicant agrees to comply with City of Tucson Floodplain and Erosion Hazard Regulations, and is bound by the conditions stated herein. Violation of any of the conditions stated or referred to below, as determined by the City Engineer, shall be grounds for revocation of this permit. Responsibility for renewal of this permit, if necessary, is with the applicant. Issuance of this permit does not constitute a waiver of other requirements contained in the Tucson City Code, Land Use Code, or any other Federal, State, or local regulation. No use other than as permitted herein is allowed. Additional uses allowed under City of Tucson Floodplain and Erosion Hazard Area Regulations require application for, review and approval of an additional Floodplain Use Permit. The applicant attests to the fact that the plans and documents submitted represent all of the improvements that will be done to the existing building, and that all additions, improvements or repairs to the building are included in the construction plans and/or documents herewith. No other improvements, repairs, reconstruction, additions or remodeling have been made to the structure that is not indicated in the attached plans and/or documents. 1) All work shall conform to the stamped/approved plans and documents (DSD Activity #T04CM00847) sealed on 2/04, which are made part of this permit. 2) Within 5 (five) days of issuance of this permit, through completion of the project, notification must be posted and maintained in a conspicuous place on the job site stating "CONSTRUCTION PROCEEDING UNDER REQUIREMENTS OF CITY OF TUCSON FLOODPLAIN USE PERMIT # T04OT00514 - FOR INFORMATION PLEASE CALL 791-5550." 3) This permit is subject to an additional floodplain permit application and review, if any proposed or required modifications are necessary within the regulatory floodplain that may cause a need for revisions to the approved plans. 4) This improvement project will be considered a substantial improvement to an existing floodprone structure if the cumulative increase in value to the existing structure as a result of this improvement is 50% or more than the base value of the existing structure, as determined by Development Services Department. If the project is determined to be a substantial improvement, and it is determined that the lowest floor elevation is below the regulatory flood elevation, a design submittal, review, and acceptance of the submittal for floodproofing will be required, and shall conform to current City of Tucson and FEMA floodplain development requirements in effect on the date of authorization by Development Services Department. 5) Except as approved by the City Engineer prior to the start of construction, development under this permit shall not obstruct, retard or divert the existing flow of floodwaters from or to adjacent properties. 6) A right-of-way permit is required for any development, excavation, or construction activity that may be necessary within the public right of way. Contact Tucson Department of Transportation, Engineering Division - Permits and Codes Section at 791-5100 for questions relating to right of way permit information. This permit does not authorize any development, excavation or construction activity in the public right-of-way.
