THERE WAS A GREASE FIRE THAT HAD OCCURRED INSIDE THE KITCHEN AREA WHICH LIGHTING FIXTURES AND THEIR ENERGIZING CONDUCTORS HAVE ENDURED HEAT DAMAGE. THE PLUMBING SUPPLYING GAS TO THE KITCHEN'S APPLIANCES HAVE ENDURED HEAT DAMAGE. THE KITCHEN'S HOOD SUPPRESSION SYSTEM FAILED TO ACTIVATE AND NEEDS TO BE RECHARGED UNDER APPLICABLE PERMITS. THERE WAS NO DAMAGE TO THE BUILDING FROM THE FIRE. DURING THE EMERGENCY INSPECTION: MULTIPLE CODE VIOLATIONS HAVE BEEN IDENTIFIED TO THE ELECTRICAL INFRASTRUCTURE THAT WAS NOT DUE TO THE FIRE. MAIN ELECTRICAL SERVICE PANEL IS MISSING THE GROUNDING CONDUCTORS TO THE GROUNDING RODS. THERE IS AN INTERIOR DOWNSTREAM PANEL WITH EXCESSIVE RUST, UNUSED OPENINGS, MULTIPLE JUNCTION BOXES MISSING COVERS, MULTIPLE RECEPTACLES MISSING COVERS, AND MULTIPLE CONDUIT RUNS MISSING HARDWARE TO KEEP SECURE. THERE ARE MULTIPLE OPEN DRY WALL PENETRATIONS THROUGHOUT THE INTERIOR CEILING OF THE KITCHEN FROM APPARENT ROOF LEAKS. NOTICE OF VIOLATION HAS BEEN ISSUED TO THE BUSINESS OWNER, OWNER HAS BEEN ORDERED TO KEEP THE POWER OFF AND SECURED UNTIL REPAIRS HAVE BEEN MADE WITH APPLICABLE PERMITS. OWNER ACKNOWLEDGE AND SIGNED ALL DOCUMENTS AND PLACED A LOCK ON THE SWITCH GEAR PANEL AT THE OFF POSITION.
04/22/2022 COT - ENFC - Code Enforcement - Initial SLAIWA Approved Entered by SLAIWA - On site, met with TFD and inspector RG, plumbing system: heat damage to the gas utility plumbing near the kitchen appliances. Mechanical equipment: signs of heat and smoke damage with accumulation of grease. All interior covering, walls and ceiling: multiple holes, gaps and leaks in ceiling and walls from previous of fire, electrical panel is missing securing hardware with multiple openings. Electrical system: heat damaged electrical conduit and lighting fixtures, electrical grounding conductor " bond" in missing from the service disconnect, protective covers missing in junction boxes. Gave owner 0 days to a reconnect permit, repair or replace a compromised gas plumbing. 30 days for the rest of the repairs with proper permits if needed. There was no need to condemn the property. Notice of violation was hand delivered to the manager "owner of this location" and copy of the notice of violation was mailed to the property owner.
