Drillmax did not sweep asphalt or clean site. Notified Brian Ripley stated the trailer broke down. Notified him no matter what cold patch must be placed on asphalt cut outs no exceptions. Had crew pull out of roadway until 0900 for traffic control set up per traffic control plans R2.

Asked for permission to work until 1700. I gave the ok due to open trenches needing backfill. Notified them per plans time is 0900-1530 and starting 6/23 they must abide those times per the permit. Low traffic street.

All asphalt from bore has been hot patched per city standards. Landscaping has been restored. Site appears to pass ROW inspections according to plans. Notified Steve M of UA the project manager for site near SWF pole that work is completed until fiber run to pole.

Trench that was cold patched must be replaced according to Steve m the project manager for UA. Measurements were made and Dan Livingston contacted for paving. Brian will schedule the pave and email confirmation.

Verizon wants to have a meeting with UA inspector about repair due to core ripping out the asphalt at a later date and replacing it.
