Arrived and all the units were on site and ID was on site taking pictures. The injuried man was transported to the hospital at this time. There was a hole near a water pipe that had a valve for the water. The pipe was leaking and in the process of trying to find the leak. They were digging and the leak got bigger and was going to turn the water off and the person by the hole fell into the water. He was holding a shovel in the water at the time of the incident. The management was served a notice of Work with out permits for the work being done and found no permits issued. There was no Blue Stake marks in the area at this time. T.E.P. was called to have the area checked if any line of electrical were buried in the area. The were not able to find there lines in the area and did mark where the T.E.P. lines are from the transformere to the pole. All the electrical items on the property were working at the time while on site. The management was served the N.O.V. for work being done with out permits and securing the area. Once served left the site. RV. |
