05/07/2021 TRSP - ROW - Recurring Re-inspection required TDOT-EXCAV GENERAL INSPECTION - Upon arrival at sight went to check on sidewalk pour. Checked Wolverine Trucking work previously done yesterday and noticed a little depression on a panel and concrete on scorejoint has popcorn like concrete. Spoke to Dan Robertson and send him pictures of work and also didn’t approve. Spoke to Wolverine Trucking crew on sight to remove and replace and they stated they would not redo the work and would pack up and leave. Then Spoke to Super Brandon and he noticed errors on sidewalk and stated he would have Wolverine Trucking remove and replace both panels and fix errors. Chain link fence was placed over area of work and Wolverine Trucking stated again they were leavening the sight.

John stated they would start on corrections work and do what they could today.

05/05/2021 TRSP - ROW - Recurring Re-inspection required TDOT-EXCAV GENERAL INSPECTION - Got together with Brandon super with Wolverine Trucking and spoke about work to be completed for ROW on this permit to be finalized, we agreed and Brandon stated would try to have crews here to start on sidewalk and cleanup today if not by Friday to then install new pole. Brandon got notified about saw cutting sidewalk on score joint and not in middle of panel like theypreviously did and told me he would correct error.

One of the panels is sawcut at middle of panel, crew to sawcut at score joint. Area of work to be beautified and future pole to be installed. Spoke to crew and stated they would pour concrete tomorrow at noon for foundation and slurry if they were done with installation of conduit. All excavated area secured with chain link fence. Crew to install one panel of chain link fence to lock excavated area once they are done with placing conduit in trench.

No concerns hydrovac on foundation will start tomorrow crews never came back and called Quail construction to set traffic control aside. Also set a warning striped cone on screw and bolts attached of removed pole and a type 2 barricade on top of cone eliminating trip hazard.
