05/16/2017 | FIRE INSP - STATE LICENSE | Denied | 1. IFC 605.8 Temporary wiring. Temporary wiring for electrical power is allowed for a period of 90 Days. Temporary wiring methods shall meet the applicable provisions of NFPA 70. Violation: Front Door has wiring that is hanging down, is not in conduit and is not of proper workmanship. 2. IFC 605.2 Work Space Illuminations. Illumination shall be provided for service equipment areas, motor control center and electrical panelboard. Violation: Riser Room shall have a light installed for illumination. 3. IFC 1011.3 Exit Sign Illumination. Exit signs shall be internally or externally illuminated. Violation: Exit light in the hallway south of room 137 needs to be repaired or replaced, internal light did not work when not in emergency power. 4. IFC 1011.1 Ext Sign Where Required. Exit door shall be marked. Violation: Exit sign shall be placed in the hallway near Room 169 5. IFC 901.4.5 Appearance of Equipment. Any device that has the physical appearance of life safety or fire protection equipment but that does not perform that fire protection function shall be prohibited. Violation: Old sprinkler system in room 170 shall be removed and system capped. Shall remove pull station and Horn Strobe in Room 159. 6. IFC 703.1 Fire-Resistance-Rated Construction Maintenance. Openings made therein for passage of pipes, electrical conduit, wires, ducts, and holes made for any reason shall be protected with approved methods. Violation: Room 160 has penetration in the ceiling and shall be sealed with an approved method. Room 133 shall have ceiling tile replaced. 7. IFC 912.3 Fire Department Connection Access. Immediate access to the fire department connections shall be maintained at all times and without obstruction by fences, bushes, trees, walls or any other fixed or moveable object. Violation: Bushes near the Fire Department Connection (FDC) shall be removed. There shall be a sign placed over the Fire Department Connection that stated FDC. 8. IFC 907.8.1 Fire Alarm Maintenance Required. Whenever required for compliance with the provisions of this code, devices equipment, system, conditions, arrangement, levels of protection or other features shall be continuously maintained. Violation: Water Flow Alarm is mounted on a board that is dilapidated and shall be replaced. 9. IFC 906.7 Portable Fire Extinguishers Hangers and Brackets. Hand-held portable fire extinguishers not housed in cabinets, shall be installed on the hanger or brackets supplied. Hangers or brackets shall be securely anchored to the mounting surface in accordance with the manufacture's installation instruction. Violation: Fire Extinguisher shall be re-mounting in Room 160 10. IFC 901.6 Fire Protection System Inspection, Testing and Maintenance. Fire detection and alarm systems, fire-extinguishing system shall be maintained in an operative condition at all times. Violation: Sprinkler head will not met the required coverage for the room it is over 11ft from the wall and need to be moved or another head will need to be added. 11. IFC 113 Amendment Table 113 - Permit Fee Table. An operational permit shall be obtained for a state licensed facility. Violation: You shall submit a permit application and pay the required permit fee before the operational permit for a state licensed facility will be issued. |
