05/25/2017 FIRE INSP - STATE LICENSE Denied 2012 IFC 703.2 Opening protectives. Opening protectives shall be
maintained in an operative condition in accordance with NFPA 80. Where allowed by the fire code official, the application of field-applied labels associated with the maintenance of opening protectives shall follow the requirements of the
approved third-party certification organization accredited for listing the opening protective. Fire doors and smoke barrier doors shall not be blocked or obstructed, or otherwise made inoperable. Fusible links shall be replaced promptly whenever fused or damaged. Fire door assemblies shall not be
modified.
1) You shall remove paint from UL listed tags on all fire door frames and on all fire doors that have been covered by paint. Where smoke seals are necessary to stop the passage of smoke, they shall be installed. Doors and door assemblies with no UL tag or a tag that is not legible, shall be either replaced or certified as required in the above code reference: West fire doors, no UL plate, East fire doors need smoke seal, replace missing listed fire door to 'GI'.

2012 IFC 605.1 Abatement of electrical hazards. Identified electrical hazards shall be abated. Identified hazardous electrical conditions in permanent wiring shall be brought to the attention of the responsible code official. Electrical wiring, devices, appliances and other equipment that is modified or damaged and constitutes an electrical shock or fire hazard shall not be used.
1) You shall place a GFI outlet over heandwash area in peds ( currently not GFI).

2012 IFC 907.6.2 Power supply. The primary and secondary power supply for the fire alarm system shall be provided in accordance with NFPA 72.
1) You shall secure the FA circuit # 35 on EASC panel by placing a lockout switch on it.






2012 IFC 901.6 Inspection, testing and maintenance. Fire detection, alarm, and extinguishing systems, mechanical smoke exhaust systems, and smoke and heat vents shall be maintained in an operative condition at all times, and shall be replaced or repaired where defective. Nonrequired fire protection systems
and equipment shall be inspected, tested and maintained or removed.
1) You shall place a sign indicating where the inspectors test valve is located ( currently blocked by shelf).
2) There are three sprinkler heads in procedure room #3 that have recessed heads. You shall have a certified person or company make the necessary repair to cause these heads to be positioned in the ceiling as per NFPA 13.
3) You shall replace the missing escutcheon plate on the head in room # 4

2012 IFC 1030.2 Reliability. Required exit accesses, exits and exit discharges
shall be continuously maintained free from obstructions or impediments to full instant use in the case of fire or other emergency when the building area served by the means of egress is occupied. An exit or exit passageway shall not be
used for any purpose that interferes with a means of egress.
1) You shall remove the refridgerator and radiation cabinets from blocking the exit door in the OR ( near the front desk of OR).
