03/02/2017 FIRE INSP - STATE LICENSE Denied 2012 IFC 907.8.3 Smoke detector sensitivity. Smoke detector sensitivity shall be checked within one year after installation and every alternate year thereafter. After the second calibration test, where sensitivity tests indicate that the detector has remained within its listed and marked sensitivity range (or 4- percent obscuration light grey smoke, if not marked), the length of time between calibration tests shall be permitted to be extended to a maximum of five years.
1) Your fire alarm report stated that your smoke detectors may be due for sensitivity testing. You shall have the testing completed if your facility is in need of such testing based upon the above cited code.

2012 IFC 1008.1.9.7 Delayed egress locks. #5. A sign shall be provided on the door located above and within 12 inches (305 mm) of the release device reading:
PUSH UNTIL ALARM SOUNDS. DOOR CAN BE OPENED IN 15 [30] SECONDS.
1) Your alarm sounds at 30 seconds, however your signage stated it could be opened in 15 seconds. Your signage shall correlate correctly with the action of your delayed egress lock.

2012 IFC 703.2 Opening protectives. Opening protectives shall be maintained in an operative condition in accordance with NFPA 80. Where allowed by the fire code official, the application of field-applied labels associated with the maintenance of opening protectives shall follow the requirements of the
approved third-party certification organization accredited for listing the opening protective. Fire doors and smoke barrier doors shall not be blocked or obstructed, or otherwise made inoperable.






(cont'd) - Fusible links shall be replaced promptly whenever fused or damaged. Fire door assemblies shall not be modified.
1) You shall remove paint covering the UL listing of your fire doors in the #100 hall.
2) You shall remove all 'foot pedal' hold open devices that are attached to your fire doors throughout the entire facility. These are not approved hold open devices.

2012 IFC 315.3.3 Equipment rooms. Combustible material shall not be stored in boiler rooms, mechanical rooms or electrical equipment rooms.
1) You shall remove all combustible materials stored in your mechanical rooms.

2012 IFC 1006.1 Illumination required. The means of egress, including
the exit discharge, shall be illuminated at all times the building space served by the means of egress is occupied.
1) You shall cause your Emergency Light in the #400 mechanical room to operate at all times ( on primary building power and when primary power is out)

2012 IFC 901.6 Inspection, testing and maintenance. Fire detection,
alarm, and extinguishing systems, mechanical smoke exhaust systems, and smoke and heat vents shall be maintained in an operative condition at all times, and shall be replaced or repaired where defective. Nonrequired fire protection systems and equipment shall be inspected, tested and maintained
or removed.
1) You shall replace missing escutcheon plate in room #101.
