Fire detection, alarm, and extinguishing systems, mechanical smoke exhaust systems, and smoke and heat vents shall be maintained in an operative condition at all times, and shall be replaced or repaired where defective. Nonrequired fire protection systems and equipment shall be inspected, tested and maintained or removed. 1) You shall replace missing escutcheons in OR #1 and in soiled linen. 2) You shall have no gap between the ceiling and the escutcheon plates in stairwell D. 2012 IFC 605.10 Portable, electric space heaters. Exception: The use of portable, electric space heaters in which the heating element cannot exceed a temperature of 212°F (100°C) shall be permitted in nonsleeping staff and employee areas in Group I-2 occupancies. 1) In Picu-Peds ED, all space heaters shall be of the type where they shut off when tipped. There were two found that shall be replaced with approved heaters or removed. 2012 IFC 5303.5.3 Securing compressed gas containers, cylinders and tanks. Compressed gas containers, cylinders and tanks shall be secured to prevent falling caused by contact, vibration or seismic activity. 1) You shall secure your compressed gas cylinders in Gyn Surgery. Upon inspection there were 8 unsecured bottles. Your amounts of oxidizing gas were in your MAQ's for the first floor level. |

The face of an exit sign illuminated from an external source shall have an intensity of not less than 5 foot candles (54 lux). 1) You shall cause the self-luminous exit sign at the entrance inside the gym to have a least 5 foot candles. Current placement does not allow the sign to absorb enough light to self-illuminate. 2012 IFC 605.6 Unapproved conditions. Open junction boxes and open-wiring splices shall be prohibited. Approved covers shall be provided for all switch and electrical outlet boxes. 1) You shall place an approved cover on the open j-box on the ceiling at the entrance to the gym. 2012 IFC 5704.3.2.1.3 Doors. Doors on a flammable liquid storage cabinet) shall be well fitted, self-closing and equipped with a three point latch. 1) You shall ensure the flammable liquid storage cabinet located in on level 5 of 'the tower' meets the above requirement. 2012 IFC 5003.8.6.1 Construction. Gas cabinets shall be constructed in accordance with the following: Be provided with self-closing doors. 1) Your gas cabinets housing 'E' size oxygen bottles on the 4th floor of the 'Tower' shall have doors that are self-closing. These cabinets are necessary to have the maximum allowed quantity and there for are required. 2012 IFC 5003.1.1(1) Maximum Allowable Quantities & 2012 IFC 5003.8.3.2 Design and Number of Control Areas. Your maximum allowable quantity of an oxidizing gas in storage in one control area (in cabinets & with a fully sprinklered building is 6000 cubic feet). You are only allowed 12.5 % of the MAQ in one control area on the fourth floor above grade, which reduces your allowable amounts to 750 cubic feet (in approved gas cabinets and fully sprinklered building). The code allows 2 control areas on the fourth floor. 1) You shall reduce the amount of oxygen in storage on the fourth floor in each control area so that it meets these requirements. 2012 IFC 703.1 Maintenance. The required fire-resistance rating of fire-resistance-rated construction (including walls, fire-stops, shaft enclosures, partitions, smoke barriers, floors, fire-resistive coatings and sprayed fire-resistant materials applied to structural members and fire-resistant joint systems) shall be maintained. 1) You shall repair the break in the fire rated wall in the soil utility across #4015 (SW). 2) You shall remove the paint from the UL listing tag on the door in the North hall. |

Exterior doors and their function shall not be eliminated without prior approval. Exterior doors that have been rendered nonfunctional and that retain a functional door exterior appearance shall have a sign affixed to the exterior side of the door with the words THIS DOOR BLOCKED. The sign shall consist of letters having a principal stroke of not less than 3/4 inch (19.1 mm) wide and at least 6 inches (152 mm) high on a contrasting background. 1) You have a lobby door by your aquarium that is not functional, but bears the appearance of a functional door. You shall make it functional, or remove all hardware so as to not appear as a door and place a sign on the outside as per above. 2012 IFC 605.6 Unapproved conditions. Open junction boxes and open-wiring splices shall be prohibited. Approved covers shall be provided for all switch and electrical outlet boxes. 1) You shall place an approved cover over the open j-box in ceiling in ED waiting area. Gas cabinets shall be self-closing. 1) The gas cabinet in the ED North is not latching after closing when held from the wide open position. (fixed on site) 2012 IFC 1008.1.9 Door operations. Except as specifically permitted by this section egress doors shall be readily openable from the egress side without the use of a key or special knowledge or effort. 1) You shall remove the key locking cylinder from the 'egress' side of the patio gate outside near Ambulance entrance. This is part of the building egress system for that hallway. 2012 IFC 1030.2 Reliability. Required exit accesses, exits and exit discharges shall be continuously maintained free from obstructions or impediments to full instant use in the case of fire or other emergency when the building area served by the means of egress is occupied. An exit or exit passageway shall not be used for any purpose that interferes with a means of egress. 1) You shall cause the exit door South of the ED administration offices to operate without sticking. 2012 IFC 901.6 Inspection, testing and maintenance. Fire detection, alarm, and extinguishing systems, mechanical smoke exhaust systems, and smoke and heat vents shall be maintained in an operative condition at all times, and shall be replaced or repaired where defective. Currently, it is next to your entrance to the kitchen. 2012 IFC 1011.1 Where required. Exits and exit access doors shall be marked by an approved exit sign readily visible from any direction of egress travel. 1) You shall install an exit sign near the soffit in the kitchen area so that employees near the dish washing can identify their path to an exit. 2) You shall install an exit sign over the exit door from the kitchen dry storage area. 2012 IFC 1030.2 Reliability. An automatic sprinkler system shall be provided throughout buildings with a Group I fire area. 1) You shall provide sprinkler coverage for the sage brush storage closets 2012 IFC 703.1 Maintenance. The required fire-resistance rating of fire-resistance-rated construction (including walls, fire stops, shaft enclosures, partitions, smoke barriers, floors, fire-resistive coatings and sprayed fire-resistant materials applied to structural members and fire-resistant joint systems) shall be maintained. 1) You shall repair/replace missing mono-kote (fire resistive coating) on I beam in the generator switch room in Marshall Generator closet. 2) You shall provide correct fire rated sealant for perforation in the Gift shop storage exterior wall that was made for communication wires. 2012 IFC 1030.2 Reliability. 01/18/2017 | FIRE INSP - STATE LICENSE | Denied | Units 420-500 On January 18, 2017 a fire safety inspection was conducted at the above location. The following violations were discovered and shall be corrected to gain compliance with the 2012 Tucson Fire Code. 2012 IFC 903.3.3 Obstructed locations. Automatic sprinklers shall be installed with due regard to obstructions that will delay activation or obstruct the water distribution pattern. Automatic sprinklers shall be installed in or under covered kiosks, displays, booths, concession stands or equipment that exceeds 4 feet (1219 mm) in width. Not less than a 3-foot (914 mm) clearance shall be maintained between automatic sprinklers and the top of piles of combustible fibers. 1) The South most telephone wiring board in the communications / phone equipment room is blocking sprinkler coverage to the South side and the East and West rows of switch boards are blocking coverage in the center aisle. You shall have a qualified sprinkler installation company apply for the appropriate permits and submit plans for the movement or addition of sprinkler heads necessary to accomplish the coverage before work begins. 2) There are two storage alcoves in the corridor of 500. You shall either place a head in these areas, remove the curtain in front that is blocking coverage of the heads in the corridor, or proved a curtain with mesh at the top that meets the requirements of NFPA 13 in providing room for sprinkler head coverage. 3) Missing sprinkler head under canopy outside exit from 500's. 2012 IFC 1011.6.3 Power source. Exit signs shall be illuminated at all times. 1) Exit sign in "Transportation Dead Storage" is not lit on the East side of the sign. You shall cause this exit sign to operate as required above. 2) Exit sign in "Transportation Dispatch" shall be replaced or caused to operate as required above. 3) Exit sign on "Eagle Patio" in #500's is not lit 2012 IFC 906.6 Unobstructed and unobscured. Portable fire extinguishers shall not be obstructed or obscured from view. In rooms or areas in which visual obstruction cannot be completely avoided, means shall be provided to indicate the locations of extinguishers. 1) You shall place a sign above the extinguisher in your "Pharmacy IV Anti-room" as it is located in an obscured place, although properly mounted. 2012 IFC 1008.1.9 Door operations. Except as specifically permitted by this section egress doors shall be readily openable from the egress side without the use of a key or special knowledge or effort. 1) The Pharmacy Warehouse has a walk-in cage for storage of regulated high security Medications. The door to this cage has a double sided key locking cylinder. You shall remove the key locking cylinder from the egress side of the cage. Security of those medications stored in the cage shall be accomplished without compromising exiting from the cage and must meet the exiting requirements of chapter 10 of the 2012 International Fire Code. 2012 IFC 703.1 Maintenance. The required fire-resistance rating of fire-resistance-rated construction (including walls, fires tops, shaft enclosures, partitions, smoke barriers, floors, fire-resistive coatings and sprayed fire-resistant materials applied to structural members and fire-resistant joint systems) shall be maintained. 1) There is a missing ceiling tile in the #420's communications closet that was not replaced following work being done. This is an intrigue part of the fire resistance of that room and shall be replaced each time work for repairs, modifications and upgrades is made to the system. 2) Missing escutcheon plate from sprinkler head in "Trash room" #500's. 2012 IFC 703.1.2 Smoke barriers and smoke partitions. Required smoke barriers and smoke partitions shall be maintained to prevent the passage of smoke. All openings protected with approved smoke barrier doors or smoke dampers shall be maintained in accordance with NFPA 105. 1) You shall replace the doors between Units #420 and #500 which are required to be labeled and are missing the UL listing. NOTE: There is an exterior door to the loading dock from the pharmacy warehouse that bears the appearance of a fire rated door, but has not UL listing tag. There is also a roll up door beside it that has no fusible link. Further documentation or plans shall be provided by TMC indicating what code was in place at the time these doors were installed and approved. If these doors met the requirements of the code at the time of installation, were approved and have not been replaced or modified, they shall be in compliance with the current fire code for all intents and purposes. The doors may remain as they were approved until such time that opening is altered, modified or remodeled. At such time, the doors would have to comply with the current code requirements for doors in hospital corridors. |

You shall replace/repair escutcheons where missing or damaged. 2012 IFC 605.5 Extension cords. Extension cords and flexible cords shall not be a substitute for permanent wiring. Extension cords and flexible cords shall not be affixed to structures, extended through walls, ceilings or floors, or under doors or floor coverings, nor shall such cords be subject to environmental damage or physical impact. Extension cords shall be used only with portable appliances. 1) 2 extension cords behind nurses' station. You shall remove from service any extension cord used as permanent wiring. 2012 IFC 509.2 Equipment access. Approved access shall be provided and maintained for all fire protection equipment to permit immediate safe operation and maintenance of such equipment. Storage, trash and other materials or objects shall not be placed or kept in such a manner that would prevent such equipment from being readily accessible. 1) Electric Panel latch broken in #700 kitchen. You shall repair/replace handle/latch so panel is accessible to emergency personnel. 2012 IFC 1030.10 ( added in the modifications to the 2012 adopted code by amendment) Panic hardware. Where panic and fire exit hardware is installed within the required path of egress or any area accessible to the public, there shall be no other locking device on the door. 1) There is a quarter turn bolt on the exterior of an exit door from the NE entrance to the hospital near the volunteer desk, which prevents full instant use of that exit. Regardless of whether this is a required exit or not, it is accessible to the public and shall not have any other locking device on it other than the panic hardware on the door. You shall remove any additional locking device on this door. 2) There is a wheelchair parked in front of and blocking access to the left leaf of an exit door from the East exit of #650. (removed immediately) You shall educate hospital staff as to not block any exits with equipment in the future. 2012 IFC 703.1 Maintenance. The required fire-resistance rating of fire-resistance-rated construction (including walls, fire-stops, shaft enclosures, partitions, smoke barriers, floors, fire-resistive coatings and sprayed fire-resistant materials applied to structural members and fire-resistant joint systems) shall be maintained. Such elements shall be visually inspected by the owner annually and properly repaired, restored or replaced when damaged, altered, breached or penetrated. 1) There are ceiling tiles across from #961 with holes or improperly fitted to ceiling grid as to cause gaps in rated ceiling. You shall repair/replace ceiling tiles where needed. |
