I WAS TOLD OCCUPANT HAD MENTAL HEALTH ISSUES, IS THOUGHT TO HAVE STARTED A FIRE IN THE BATHROOM. OCCUPANT WAS REMOVED FROM THE APT AND TAKEN TO ST JOSEPH'S HOSPITAL WITH BURNS AND IS EXPECTED TO BE IN THE HOSPITAL FOR AT LEAST A WEEK.

MET WITH MANAGER MARY CLARK WHO STATED OCCUPANT HAS BEEN A PROBLEM PERSON HAS BEEN WARNED OF LEASE BEING DISCONTINUED IF PROBLEMS PERSISIT. MARY CONTACTED OCCUPANTS MOTHER AND INFORMED HER OF WHAT HAD HAPPENED AND ALSO INFORMED HER THAT THE LEASE WOULD BE DISCONTINUED. MOTHER STATED THAT DAUGHTER JAN SKELLY COULD STAY WITH HER WHEN RELEASED FROM THE HOSPITAL. MY FINDINGS OF THE UNIT ARE AS FOLLOWS:
>UNIT IS A ONE BDRM, ONE BATH. BATHROOM APPROX 5 FT X 8 FT. AND COMPLETELY BURNED.
>DOOR AND CABINETS COMPLETELY BURNED
>SHOWER STALL MELTED AND PLUMBING AND FIXTURES RECEIVED FIRE DAMAGE. PLUMBING LEFT EXPOSED.
>ELECTRICAL WIRING, SWITCH, LIGHT OUTLET AND FIXTURE RECEIVING FIRE DAMAGE. WIRING LEFT EXPOSED. MAIN SERVICE PANEL TURNED OFF AT MAIN DISCONNECT AND PANEL LOCKED OUT AND REMAINS OFF.
>DRYWALL IN THE BATHROOM COMPLETELY BURNED, FRAMING AND FLOOR/ CEILING STRUCTURE RECEIVED NO FIRE DAMAGE.
> NOV ISSUED FOR EMERGENCY VACATE. STRUCTURE HAS NO POWER AND SHOULD NOT BE REOCCUPIED. NOV INCLUDED SECTION FOR PERMITS REQUIRED. NOV SIGNED FOR BY MARY CLARK. UNIT POSTED UNFIT TO OCCUPY, AND FRONT DOOR LOCK AND DEAD BOLT CHANGED OUT SO ALL ENTRY WOULD REQUIRE CONTACT WITH THE MANAGER.

> CHECKED UNITS 1116, 2116, AND 2117 FOR ANY DAMAGE OF SMOKE DAMAGE. ALL UNITS OK TO OCCUPY.
