10/20/2016 | FIRE INSP - STATE LICENSE | Denied | TFC 605.6 Unapproved Conditions. Open junction boxes and open-wiring splices shall be prohibited. Approved covers shall be provided for all switch and electrical outlet boxes. 1) You shall replace smoke detector in hallway back in its j box (hanging from wiring). 2) Need j box over glass book case in library. TFC 604.5 Emergency lighting equipment. Emergency lighting shall be inspected and tested in accordance with Sections 604.5.1 through 604.5.2.1. MONTHLY TEST TFC 604.5.1 Activation test. An activation test of the emergency lighting equipment shall be completed monthly. The activation test shall ensure the emergency lighting activates automatically upon normal electrical disconnect and stays sufficiently illuminated for a minimum of 30 seconds. TFC 604.5.1.1 Activation test record. Records shall be maintained on the premises for a minimum of three years and submitted to the fire code official upon request. The record shall include the location of the emergency lighting tested, whether the unit passed or failed, the date of the test, and the person completing the test. 1) You shall perform a monthly test of every emergency light that is on day care/school property including all areas that students, children in care, staff and teachers function within routinely), documenting the location of each, pass or fail, test date, and person completing test. This record shall be maintained and presented to the fire inspector upon request at the next inspection of your facility. YEARLY TEST TFC 604.5.2 Power test. For battery-powered emergency lighting, A power test of the emergency lighting equipment shall be completed annually. The power test shall operate the emergency lighting for a minimum of 90 minutes and shall remain sufficiently illuminated for the duration of the test. TFC 604.5.2.1 Power test record. Records shall be maintained on the premises for a minimum of three years and submitted to the fire code official upon request. The record shall include the location of the emergency lighting tested, whether the unit passed or failed, the date of the test, and the person completing the test. 1) You shall perform an annual test of the battery powered emergency lighting that is on daycare/school property (including all areas that students, children in care, staff and teachers function within routinely), documenting the location of each, pass or fail, test date, and person completing test. This record shall be maintained and presented to the fire inspector upon request at the next inspection of your facility. TFC 605.4.2 Power supply. Re-locatable power taps shall be directly connected to a permanently installed receptacle. 1) You shall disconnect one power strip from another in the 'Education Office - Conference Room' and provide each with its own dedicated outlet. TFC 1011.6.3 Exit Sign Power source. Exit signs shall be illuminated at all times. To ensure continued illumination for a duration of not less than 90 minutes in case of primary power loss, the sign illumination means shall be connected to an emergency power system provided from storage batteries, unit equipment or an on-site generator. The installation of the emergency power system shall be in accordance with Section 604. (see testing procedures above in 604.5.1-604.5.2.1) Exception: Approved exit sign illumination means that provide continuous illumination independent of external power sources for a duration of not less than 90 minutes, in case of primary power loss, are not required to be connected to an emergency electrical system. 1) You shall perform monthly and annual tests of the backup power supply to your exit signs that are on daycare/school property (including all areas that students, children in care, staff and teachers function within routinely), documenting the location of each, pass or fail, test date, and person completing test. This record shall be maintained and presented to the fire inspector upon request at the next inspection of your facility. TFC 605.5 Extension cords. Extension cords and flexible cords shall not be a substitute for permanent wiring. 1) You shall remove extension cord in 'F' Room powering fish tank 2) You shall remove extension cord in 'F' Room powering CD player/ Halloween lights and phone charger. TFC 907.3.4 Wiring. The wiring to the auxiliary devices and equipment used to accomplish fire safety functions shall be monitored for integrity in accordance with NFPA 72. 1) A lock out switch shall be placed on the #17 circuit labeled FA in refrigerator room. TFC 1008.1.9 Door operations. Except as specifically permitted by this section egress doors shall be readily openable from the egress side without the use of a key or special knowledge or effort. 1) You shall remove the key cylinder lock on your door to the Sukkot. You cannot require the use of a key to exit. TFC 1008.1.9.5 Unlatching. The unlatching of any door or leaf shall not require more than one operation. 1) You shall remove the 2nd lock on the South exit door from room #5. 2) You shall remove the 2nd lock on the exit door from inside the gift shop to the exit. (You may use a dummy handle instead of one that requires a twist to unlock before it is turned to unlatch the door). TFC 1030.2 Reliability. Required exit accesses, exits and exit discharges shall be continuously maintained free from obstructions or impediments to full instant use in the case of fire or other emergency when the building area served by the means of egress is occupied. An exit or exit passageway shall not be used for any purpose that interferes with a means of egress. 1) You shall cause your SE exit door to the outside of Room #10 to operate with no extra effort or force (it is currently sticky). 2) You shall place the changing table in Room # 11 in a place where it is not blocking the exit to the hallway. TFC 609.3.3.3 Records. Records for inspections shall state the individual and company performing the inspection, a description of the inspection and when the inspection took place. Records for cleanings shall state the individual and company performing the cleaning and when the cleaning took place. Such records shall be completed after each inspection or cleaning, maintained on the premises for a minimum of three years and be copied to the fire code official upon request. 1) You shall provide the fire inspector a copy of your most recent hood suppression system inspection report and documentation of your most recent hood cleaning from filters to roof vent/fan. TFC 3317 ( e) 1976 edition.
