| 03/17/2011 | INSP - UST | Correction Required | FIRE INSPECTION REPORT 03-17-2011 Shell 880 West Saint Mary's Tucson, AZ Dear Sir or Madam, The Tucson Fire Department has conducted a fire safety inspection at the above stated business on 03-15-2011. Below is a list of violations that shall be corrected to provide compliance with the applicable codes, regulations and ordinances. Compliance is required by the 2006 International Fire Code with modifications. A Re-inspection will be performed on or about 04-04-2011. 1011.4 Internally illuminated exit signs. Internally illuminated exit signs shall be listed and labeled and shall be installed in accordance with the manufacturer's instructions and Section 2702 of the International Building Code. Exit signs shall be illuminated at all times. -Remove the Exit sign that is no longer in use. 605.5 Extension cords. Extension cords and flexible cords shall not be a substitute for permanent wiring. -Only power strips or permanent wiring may be used. 605.6 Unapproved conditions. Open junction boxes and open-wiring splices shall be prohibited. Approved covers shall be provided for all switch and electrical outlet boxes. -Replace the missing outlet covers 901.6 Inspection, testing and maintenance. Fire detection, alarm and extinguishing systems shall be maintained in an operative condition at all times, and shall be replaced or repaired where defective. -Fire extinguishers shall be inspected annually and mounted correctly. 2205.2.3 Leak detectors. Leak detection devices required by Section 2206.7.7.1 shall be checked and tested at least annually in accordance with the manufacturer's specifications to ensure proper installation and operation. Please provide documentation of test results and ensure all leak detection devices are installed as per the manufacturer. This INCLUDES the waste oil tank. If it is going to remain in ground it shall have leak monitoring. Section 3404.2.12.3 Existing tanks and piping. Existing underground storage tanks and piping shall be tested for leakage at the owner's or operator's expense at least every 24 months in accordance with this section. EXCEPTION: Underground storage tank systems with an approved method of detecting leaks from any component of the system. Tanks shall be emptied of flammable or combustible liquids if required tests are not completed within the specified time. When testing is required, owners or operators shall provide the fire code official with data setting forth the method of testing that is to be used and shall submit the name of a qualified individual who will conduct the test. A testing permit shall be obtained. The method of testing to be used shall consider the effects of temperature, pressure and other variables and shall establish conclusively whether the tank or piping is leaking. Pneumatic testing is prohibited. Devices used for final testing shall be capable of detecting leaks as small as 0.05 gallon per hour. Leaking piping and equipment shall not be used until repaired or replaced. The fire code official is authorized to require that the test be conducted in the code official's presence. Please provide a copy of either the tank tightness and line leak test results or if a automatic tank gauge system is used please provide the annual calibration test report for that device and the print out showing that the system has passed its leak test. 2205.2.2 Emergency shutoff valves. Automatic emergency shutoff valves required by Section 2206.7.4 shall be checked not less than once per year by manually tripping the hold-open linkage.
