Merlo One CVS Drive Woonsocket, RI 02895 RE: Fire Prevention Inspection: 7901 E Golf Links Tucson, AZ. Dear Mr. Merlo The Tucson Fire Department has conducted a fire safety inspections at the above stated business on September 1, 20100. At that time a notice was issued indicating the corrections required to provide compliance with the applicable codes, regulations and ordinances. A re-inspections was also conducted on September 15, 2010. None of the violations noted were corrected and we were unable to gain voluntary compliance. Compliance is required by the 2006 Tucson Fire Code: 1. _____907.1 General. This section covers the application, installation, performance and maintenance of fire alarm systems and their components in new and existing buildings and structures. The requirements of Section 907.2 are applicable to new buildings and structures. The requirements of Section 907.3 are applicable to existing buildings and structures - FIRE ALARM SYSTEM SHALL BE TESTED ANNUALLY 2. _____ 605.5 Extension Cords. Extension cords and flexible cords shall not be a substitute for permanent wiring. Extension cords and flexible cords shall not be affixed to structures, extended through walls, ceilings or floors, or under doors or floor coverings, nor shall such cords be subject to environmental damage or physical impact. Extension cords shall be used only with portable appliances. -FRONT COUNTER 3. _____ 605.3 Working space and clearance. A working space of not less than 30 inches (762 mm) in width, 36 inches (914 mm) in depth and 78 inches (1981 mm) in height shall be provided in front of electrical service equipment. Where the electrical service equipment is wider than 30 inches (762 mm), the working space shall not be less than the width of the equipment. No storage of any materials shall be located within the designated working space. - NEAR REAR RISER ROOM 4. _____ 1006.3 Illumination emergency power. The power supply for means of egress illumination shall normally be provided by the premise's electrical supply. In the event of power supply failure, back-up power shall be supplied -Provide back up power supply for all required exit illumination: - 2 EACH IN REAR STOCK ROOM - EXIT SIGN NEAR NORTHEAST EXIT DOOR 5. _____ 703.1 Maintenance. The required fire-resistance rating of fire-resistance-rated construction (including walls, fire stops, shaft enclosures, partitions and floors) shall be maintained. Such elements shall be properly repaired, restored or replaced when damaged, altered, breached or penetrated. - REPLACE MISSING CEILING TILE NEAR REAR PHAMACY DOOR 6. _____ 605.6 Unapproved conditions. Open junction boxes and open-wiring splices shall be prohibited. Approved covers shall be provided for all switch and electrical outlet boxes. - ON THE POLES IN THE MIDDLE OF THE STORE 7. _____ 901.4 Installation./ NFPA 2008 25 Section 9-4.1.2 Fire protection systems shall be maintained in accordance with the original installation standards for that system. Systems shall be extended, altered, or augmented as necessary to maintain and continue protection whenever the building is altered, remodeled or added to. Alterations to fire protection systems shall be done in accordance with applicable standards. - MAINTAIN 4 FEET CLEARANCE AROUND SPRINKLER RISER This letter is to inform you that if the above stated necessary corrections have not been completed September 28, 2010 a citation will be issued which will require an appearance by you in court with a probable fine.
