Dear Mr. De Shetler

The Tucson Fire Department has conducted a fire safety inspection at the above stated business on October 22, 2009. Below is a list of violations that shall be corrected to provide compliance with the applicable codes, regulations and ordinances. Compliance is required by the 2006 International Fire Code (with modifications). A Re-inspection will be performed on or about November 5, 2009.

Notice of Violations:
1. _____ 1006.3 Illumination emergency power. The power supply for means of egress illumination shall normally be provided by the premise's electrical supply. In the event of power supply failure, back-up power shall be supplied.

- Required Correction: Check ALL emergency light and ensure they are operating correctly

2._____ 605.5 Extension Cords. Extension cords and flexible cords shall not be a substitute for permanent wiring. Extension cords and flexible cords shall not be affixed to structures, extended through walls, ceilings or floors, or under doors or floor coverings, nor shall such cords be subject to environmental damage or physical impact. Extension cords shall be used only with portable appliances:

- Required Correction:
i. Remove extension cords:
1. Near front door going through ceiling.
2. Going through ceiling above bar area
3. Rear storage area (dressing room)
4. Going through the wall in the outside storage area

ii. Replace ceiling tile above aisle 7C

3.____ 901.6 Inspection, testing and maintenance. Fire detection, alarm and extinguishing systems shall be maintained in an operative condition at all times, and shall be replaced or repaired where defective. Non-required fire protection systems and equipment shall be inspected, tested and maintained or removed.

- Required Correction:
i. Fire Alarm system shall be tested and repaired or removed.
ii. Fire extinguisher shall be tested in the bar area
iii. Fire extinguisher shall be serviced in the kitchen area

4. _____ 904.11.6.4 Extinguishing system service. Automatic fire-extinguishing systems shall be serviced at least every 6 months and after activation of the system. Inspection shall be by qualified individuals, and a certificate of inspection shall be forwarded to the fire code official upon completion.

- Required Correction: Hood system shall be serviced by a qualified person and a certificate of inspection provided.

5. _____ 605.4 Multiplug adapters. Multiplug adaptors, such as cube adaptors, unfused plug strips or any other device not complying with the ICC Electrical Code shall be prohibited.
- Required Action:
i. Remove the unapproved multi-cube adapter above the bar area
ii. Replace the missing electrical cover-plate behind the dart machine
iii. Replace the electrical cover-plate under the bar under (East wall)


6._____ 1028.6 Panic Hardware. Where panic hardware is installed, the panic or fire exit hardware shall be the only locking device on the door…
- Condition: Currently there is are double-key locking hardware on the two exit door provided with panic hardware in the "Smoking Premises Extension area."

- Required Action: Remove the double-key locking hardware.

7._____ bolts are not permitted.
- Required Action: Remove the 3 flush bolts on the exit next to the stage
