Dear Mrs. Olivas,

The Tucson Fire Department has conducted a fire safety inspection at the above stated business on October 21, 2009. Below is a list of violations that shall be corrected to provide compliance with the applicable codes, regulations and ordinances. Compliance is required by the 2006 International Fire Code (with modifications). A Re-inspection will be performed on or about November 4, 2009.

Notice of Violations:
1. _____ 1006.1 Illumination required. The means of egress, shall be illuminated at all times the building space served by the means of egress is occupied.

- Required Correction: Illuminate, in an approved manner, all exits required by the Building Official
i. East exit sign shall be illuminated
ii. West exit sign shall be illuminated
iii. South Exit sign shall be illuminated


2._____ 605.5 Extension Cords. Extension cords and flexible cords shall not be a substitute for permanent wiring. Extension cords and flexible cords shall not be affixed to structures, extended through walls, ceilings or floors, or under doors or floor coverings, nor shall such cords be subject to environmental damage or physical impact. Extension cords shall be used only with portable appliances:
- Required Correction:
i. Remove extension cord going through the ceiling in the dining area.

3._______ 904.11.1 Manual system operation. A manual actuation device shall be located at or near a means of egress from the cooking area a minimum of 10 feet (3048 mm) and a maximum of 20 feet (6096 mm) from the kitchen exhaust system. The manual actuation device shall be installed not more than 48 inches (1200 mm) nor less than 42 inches (1067 mm) above the floor and shall clearly identify the hazard protected. The manual actuation shall require a maximum force of 40 pounds (178 N) and a maximum movement of 14 inches (356 mm) to actuate the fire suppression system.
- Required Action: Currently the manual activation device is hidden behind a freezer:
i. Maintain clear and free access to the manual activation device
ii. Train all employees on all shift where it is located and how to activate it.
