Met with Tom to receive check and him copy of letter usually sent through mail. He signed copy and I received check made to COT. Went over follow up inspection scheduled for 11-19-09 and will check status of permit through Permits Plus.
11/06/2009 INSP - FOLLOW UP CODE ENFORCE Approved I was passing the location on 11-5-09 and the business appeared to be opened. I went to the location with Jim Hurd on 11-06-09 and met with the manager who was trying to open. The business was still closed at the time of visit and was let in by an employee. We met with Jim Owen a manager and he called the owner Tom Palomares on the cell. We were given permission to look around. Since my last inspection there was no work done. I was waiting for the follow up on 11-19-09. All work was completed and was told they could open but no name was given. I did go to D.S.D. and all who had to talk to was gone or off. They did have the old C of O but under the old name. The name change is part of the new permit. In meeting with Tom, it was agreed to give an N.O.V. on work with out permits and pay the re-fee for inspection. Had a letter made after Gail made arrangements with accounting in receiving a check from Tom to pay for re-fee for inspection. I did check permits plus and all the inspection have been completed but is lacking approval from fire and zoning. I did make contact with the architect and the zoning was lacking the sidewalk in front of the property. I did advise that the sidewalk in front was poured on 11-06-09. John stated that it would be easier to get approval from zoning now that the sidewalk was in. I'm being told that everything is the same on license but name change and different food being made. There a few changes on the bar area but the kitchen is the same. I have met with Tom and received the check for the re-fee on inspection today. I will be checking on the 11-19-09 for permit status and advised that he needs to push the architect to get the permit issued and inspection completed as soon as possible. Will follow up in 60 days.

10/19/2009 INSP - FOLLOW UP CODE ENFORCE Approved Checked Permits Plus and permit has been applied for but not completed. T09CM02686 applied for on10-05-09 and is lacking zoning and fire approvals. Plans are out for correction and will be re-submitted this week. Will be checking in 30 days.
