THE COMBUSTION CHAMBER OF THE HEATING UNIT WAS FOUND TO BE CORRODED AND PLUGGING SOME OF THE ORAFICES FOR INTRODUCING AIR INTO TO FLAME CAUSING A HIGH PRODUCTION OF CO.
06/16/2008 INSP - LIFE SAFETY Approved AFTER MODIFICATIONS TO THE VENTILATION/EXHAUST SYSTEM IN COMPLIANCE WITH THE MECHANICAL ENGINEER'S SUGGESTIONS AND BEING ACCEPTABLE TO THE MECHANICAL INSPECTOR READINGS WERE TAKEN THROUGHOUT THE BUILDING FOR CO. THROUGHOUT THE CUSTOMER LOBBY AND BACK ROOM CO READINGS WERE BELOW 5 PPM. AROUND THE KETTLE EXHAUST HOOD READINGS WERE 10-12 PPM. UNDER THE HOOD READINGS SPIKED AT 25 PPM UNTIL THE WATER REACHED ITS MAX. TEMP., WITH THE HEATING UNIT CYCLING ON AND OFF TO MAINTAIN THE TEMP. THE READINGS VARIED FROM 2 PPM TO 24 PPM. NIOSH MANUAL SPECS. SHOW THE TWA FOR CO AS 35 PPM AND OSHA TWA IS 50 PPM. THESE READINGS ARE WILL WITHIN PERMISSABLE LEVELS. THE OVEN AND KETTLE ARE SAFE FOR NORMAL OPERATIONS. THE RED TAGS WERE REMOVED BY ME.
06/09/2008 INSP - LIFE SAFETY Violation THIS WAS A FIRE SUPPRESSION RESPONSE IN MY INSPECTION AREA. THE I.C., BATTALION CHIEF RAY KLEIN, REQUESTED A FIRE PREVENTION INSPECTOR. I RESPONDED TO THE SCENE TO FIND SW GAS AND A C.O.T. MECHANICAL INSPECTOR ON THE SCENE. THE TFD UNITS HAD MONITORED SEVERAL BUSINESSES BESIDE AND ABOVE THIS ADDRESS AND FOUND CARBON MONOXIDE LEVELS ABOVE ACCEPTABLE LEVELS AND HAD ALREADY EVACUATED EVERYONE IN THE AFFECTED AREAS. THE MECHANICAL INSPECTOR HAD INDENTIFIED 3 POSSIBLE SOURCES FOR THE CO CONTAMINATION. THE WATER HEATER, A COMMERCIAL OVEN AND A KETTLE BOILER. THE BUILDING HAD BEEN VENTILATED AND METERS WERE READING BELOW 10 PPM. SW GAS ANDTHE MECHANICAL INSPECTOR, JIM MILITELLO, WERE INVESTIGATING THE OVEN WITHOUT ANY EXECESSIVE READINGS. WE DECIDED TO TURN ON THE KETTLE BOILER HEATING UNIT AND METER THE RESULTS. THE METER WAS WITHIN 5 FEET OF THE HEATING UNIT AND WITHIN APPROXIMATELY 1 TO 2 MIN. WE WERE GETTING READINGS APPROACHING 3,000 PPM. WE SHUT IT DOWN AND LET THE BUILDING VENTILATE NATURALLY. FIRE CREWS MONITORED ADJACENT UNITS TO ENSURE THEY WERE STILL CLEAR. MR. MILITELLO DIDN'T HAVE A MEANS OF TAGGING THE EQUIPMENT SO I RED TAGGED BOTH THE OVEN AND THE KETTLE BOILER UNIT. ACCORDING TO MR. MILITELLO THE "MAKE UP AIR" FANS WERE NOT OPERATING, THE VENTILATION FAN ABOVE THE KETTLE WAS NOT ON AND THE DUCT WORK DESIGN FOR THE WATER HEATER AND THE OVEN WERE NOT PER CODE. HE ALSO NOTICED THAT THERE ARE VARIOUS HOLES FOR THE DUCT, ELECTRICAL AND PLUMBING THROUGH THE ADJACENT FIRE WALL ABOVE THE FALSE CEILING. THE FIREWALL IS THE EAST WALL SEPARATING BRUEGGER'S FROM THE ADJOINING BUSINESS. SINCE THE CODE REQUIREMENTS FOR THE MECHANICAL DUCT SYSTEM AND VENTILATION REQUIREMENTS ARE BEYOND MY SCOPE I SPOKE WITH BRUEGGER'S AREA MANAGER, JOHN JUERGENSMIYER, AND TOLD HIM HE WOULD HAVE TO HIRE AN APPROVED COMPANY TO BRING THE EQUIPMENT AND SYSTEMS UP TO THE REQUIREMENTS NOTED BY THE MECHANICAL INSPECTOR.
I LEFT MY CARD WITH THE MANAGER AND WITH THE MECHANICAL INSPECTOR SO WHEN THEY ARE READY TO RE-INSPECT AFTER NECESSAY REPAIRS I WILL MEET THEM AT THE SAME TIME AND REMOVE THE TAGS IF EVERYTHING IS ACCEPTABLE TO THE MECHANICAL INSPECTOR.
