Broadway Blvd. Tucson, AZ 85710 RE: Pre-Citation Letter On 1/29/08, a fire prevention inspection was conducted 6310 E. Broadway Rd. in Tucson, AZ. On 3/13/08, a fire prevention re-inspection was conducted at 6310 E. Broadway Rd. in Tucson, AZ. During the re-inspection several serious Tucson Fire Code Violation(s) were noted: 1). In the kitchen a portable stove burner was being used completely outside the coverage area of the hood system. This is a violation of IFC: 901.6 Inspection, testing and maintenance. Fire detection, alarm and extinguishing systems shall be maintained in an operative condition at all times, and shall be replaced or repaired where defective. Non-required fire protection systems and equipment shall be inspected, tested and maintained or removed. 2). The kitchen hood system pull station was blocked by a refrigeration device at the time of re-inspection. This is a violation of IFC: 904.11.1 Manual system operation. A manual actuation device shall be located at or near a means of egress from the cooking area a minimum of 10 feet (3048 mm) and a maximum of 20 feet (6096 mm) from the kitchen exhaust system. The manual actuation device shall be installed not more than 48 inches (1200 mm) nor less than 42 inches (1067 mm) above the floor and shall clearly identify the hazard protected. The manual actuation shall require a maximum force of 40 pounds (178 N) and a maximum movement of 14 inches (356 mm) to actuate the fire suppression system. 3). According to Independent Fire & Safety Inspection Report, the kitchen hood system has several deficiencies (see attached photocopy). This is a violation of the following IFC codes: 901.6 Inspection, testing and maintenance. Fire detection, alarm and extinguishing systems shall be maintained in an operative condition at all times, and shall be replaced or repaired where defective. 904.11.6.4 Extinguishing system service. Automatic fire-extinguishing systems shall be serviced at least every 6 months and after activation of the system. Inspection shall be by qualified individuals, and a certificate of inspection shall be forwarded to the fire code official upon completion. Bring your hood system into compliance immediately. This letter is to inform you that abatement of these Tucson Fire Code violations must be completed by 03-27-08. A re-inspection shall be conducted at that time to ensure compliance. Broadway Rd. in Tucson, AZ. The following Tucson Fire Code Violation(s) was noted: 1). In the liquor room there is storage on top of the hot water heater. This a violation of IFC 305.1: 305.1 Clearance from ignition sources. Clearance between ignition sources, such as luminaires, heaters, flame-producing devices and combustible materials, shall be maintained in an approved manner. Please remove storage from the top of the water heater. 2). The yearly service of your class K fire extinguisher is overdue. This is a violation of IFC 901.6: 901.6 Inspection, testing and maintenance. Fire detection, alarm and extinguishing systems shall be maintained in an operative condition at all times, and shall be replaced or repaired where defective. Please obtain required yearly service for your fire extinguisher. 3). The curtains surrounding the dance floor do not have the proper fire resistance documentation. This is a violation of IFC 807.1 807.1 General requirements. In occupancies in Groups A, E, I and R-1 and dormitories in Group R-2, curtains, draperies, hangings and other decorative materials suspended from walls or ceilings shall meet the flame propagation performance criteria of NFPA 701 in accordance with Section 806.2 or be noncombustible. Please provide documentation of fire resistance or remove curtains. 4). Fire detection, alarm and extinguishing systems shall be maintained in an operative condition at all times, and shall be replaced or repaired where defective.
