10/31/2007 | INSP - FULL | Correction Required | October 19, 2007 Jay Tisdale Building Engineer 4400 East Broadway, Suite 600 Tucson, AZ 85711 Dear Mr. Tisdale, On 10-11-07, a fire prevention inspection was conducted at the high rise building at 4400 E. Broadway in Tucson, Arizona. The following Tucson Fire Code Violation(s) was noted: 1). Candles are present in suite 811. This is a violation of IFC 308.3.2: 308.3.2 Open-flame decorative devices. Open-flame decorative devices shall comply with all of the following restrictions: 3. The device or holder shall be constructed to prevent he spillage of liquid fuel or wax at the rate of more than 0.25 teaspoon per minute (1.26 ml per minute) then the device or holder is not in an upright position. 4. The device or holder shall be designed so that it will return to the upright position after being tilted to an angle of 45 degrees from vertical. Exception: Devices that self-extinguish if tipped over and do not spill fuel or wax at the rate of more than 0.25 teaspoon per minute (1.26 ml per minute) if tipped over. Please ensure that candles in suit 811 meet aforementioned restrictions or remove them. 2). The fire alarm horn is loose in the lobby on the 8th floor. This is a violation of IFC 907.20.1: 907.20.1 Maintenance required. Whenever or wherever any device, equipment, system, condition, arrangement, level of protection or any other feature is required for compliance with the provisions of this code, such device, equipment, system, condition, arrangement, level of protection or other feature shall thereafter be continuously maintained in accordance with applicable NFPA requirements or as directed by the fire code official. Please secure fire alarm horn in lobby. 3). The fire extinguisher label is missing from a fire extinguisher cabinet on the 8th floor. This is a violation of IFC 901.6: 901.6 Inspection, testing and maintenance. Fire detection, alarm and extinguishing systems shall be maintained in an operative condition at all times, and shall be replaced or repaired where defective. Nonrequired fire protection systems and equipment shall be inspected, tested and maintained or removed. Please install fire extinguisher label on cabinet. 4). The emergency lighting unit on the 8th floor is not operational. This is a violation of IFC 1006.3: 1006.3 Illumination emergency power. The power supply for means of egress illumination shall normally be provided by the premises' electrical supply. In the event of power supply failure, an emergency electrical system shall automatically illuminate the following areas: Exterior landings, as required by Section 1008.1.5, for exit discharge doorways in buildings required to have two or more exits. Please repair or replace the emergency lighting unit on the 8th floor. 5). In suite 708 there is a missing ceiling tile in the computer room. This is a violation of IFC 703.1: 703.1 Maintenance. The required fire-resistance rating of fire-resistance-rated construction (including walls, firestops, shaft enclosures, partitions, smoke barriers, floors, fire-resistive coatings and sprayed fire-resistant materials applied to structural members and fire-resistant joint systems) shall be maintained. Such elements shall be properly repaired, restored or replaced when damaged, altered, breached or penetrated. Openings made therein for the passage of pipes, electrical conduit, wires, ducts, air transfer openings and holes made for any reason shall be protected with approved methods capable of resisting the passage of smoke and fire. Openings through fire-resistance-rated assemblies shall be protected by self- or automatic-closing doors of approved construction meeting the fire protection requirements for the assembly. Please replace ceiling tile. 6). In suite 708 there is excess storage on top of the cabinets. This is a violation of IFC 315.2.1: 315.2.1 Ceiling clearance. Storage shall be maintained 2 feet (610 mm) or more below the ceiling in nonsprinklered areas of buildings or a minimum of 18 inches (457 mm) below sprinkler head deflectors in sprinklered areas of buildings. Please remove storage from top of cabinets. 7). In suite 304 there are extension cords to the snowflake lights. This is a violation of IFC 605.5: 605.5 Extension cords. Extension cords and flexible cords shall not be a substitute for permanent wiring. Extension cords and flexible cords shall not be affixed to structures, extended through walls, ceilings or floors, or under doors or floor coverings, nor shall such cords be subject to environmental damage or physical impact. Extension cords shall be used only with portable appliances. Please remove all extension cords. 8). On the second floor there is a missing ceiling tile this is a violation of IFC 703.1: 703.1 Maintenance. The required fire-resistance rating of fire resistance-rated construction (including walls, firestops, shaft enclosures, partitions, smoke barriers, floors, fire-resistive coatings and sprayed fire-resistant materials applied to structural members and fire-resistant joint systems) shall be maintained. Combustible material shall not be stored in boiler rooms, mechanical rooms or electrical equipment rooms. Please remove combustible materials from air handler room. 10). The hood system in the A la Carte restaurant is not up to date on the 6 month service schedule. This is a violation of IFC 904.11.6.3: 904.11.6.3 Cleaning. Hoods, grease-removal devices, fans, ducts and other appurtenances shall be cleaned at intervals necessary to prevent the accumulation of grease. Cleanings shall be recorded, and records shall state the extent, time and date of cleaning. Such records shall be maintained on the premises. Please clean screen to exhaust system on roof and obtain required service and records for hood system. 11). In the A la Carte restaurant there is an extension cord used to supply a mixing stand. This is a violation of IFC 605.5: 605.5 Extension cords. Extension cords and flexible cords shall not be a substitute for permanent wiring. Extension cords and flexible cords shall not be affixed to structures, extended through walls, ceilings or floors, or under doors or floor coverings, nor shall such cords be subject to environmental damage or physical impact. Extension cords shall be used only with portable appliances. Please remove extension cords and replace with approved wiring. 12). In the General Dynamics suite there is a fire extinguisher that is not properly mounted. This is a violation of IFC 906.7: 906.7 Hangers and brackets. Hand-held portable fire extinguishers, not housed in cabinets, shall be installed on the hangers or brackets supplied. Hangers or brackets shall be securely anchored to the mounting surface in accordance with the manufacturer's installation instructions. Please mount fire extinguisher according to manufacturer's instructions. 13). In the basement gym there is an unnecessary exit sign this is a violation of IFC 1011.1: 1011.1 Where required. Exits and exit access doors shall be marked by an approved exit sign readily visible from any direction of egress travel. Access to exits shall be marked by readily visible exit signs in cases where the exit or the path of egress travel is not immediately visible to the occupants. Exit sign placement shall be such that no point in a corridor is more than 100 feet (30 480 mm)or the listed viewing distance for the sign, whichever is less, from the nearest visible exit sign. Please remove unnecessary exit sign. 14). In the basement mechanical room there is a fire extinguisher that is not properly mounted. This is a violation of IFC 906.7: 906.7 Hangers and brackets.
