The Tucson Fire Department has conducted a fire safety inspection at the above stated business on August 13, 2007. Below is a list of violations that shall be corrected to provide compliance with the applicable codes, regulations and ordinances. Compliance is required by the 2006 International Fire Code (with modifications). A Re-inspection will be performed on or about August 27, 2007

Notice of Violations:
_____ 315.2.1 Ceiling clearance. Storage shall be maintained 2 feet (610 mm) or more below the ceiling in nonsprinklered areas of buildings or a minimum of 18 inches (457 mm) below sprinkler head deflectors in sprinklered areas of buildings.
· Maintain 18" of clearance above the beer cooler.

_____ 906.7 Hangers and brackets. Hand-held portable fire extinguishers, not housed in cabinets, shall be installed on the hangers or brackets supplied
· Fire extinguisher in the liquor stock room shall be properly mounted.

_____ 1027.4 Furnishings and decorations. Furnishings, decorations or other objects shall not be placed so as to obstruct exits, access thereto, egress therefrom, or visibility thereof.
· Currently the cardboard compactor is blocking the West exit in the stock room. This shall be removed/re-located immediately.

_____ 1006.1 Illumination required. The means of egress, shall be illuminated at all times the building space served by the means of egress is occupied.
Illuminate, in an approved manner, all exits required by the Building Official
· East exit sign shall be illuminated
· West exit sign shall be illuminated

______ 407.2 Material Safety Data Sheets. Material Safety Data Sheets (MSDS) for all hazardous materials shall be readily available on the premises.
· Obtain MSDS for all chemicals.
· Train employees.
_____ 1006.3 Illumination emergency power. The power supply for means of egress illumination shall normally be provided by the premise's electrical supply. In the event of power supply failure, back-up power shall be supplied. Provide back up power supply for all required exit illumination:
· East Exit
· Emergency light above cosmetics
· Above Aisle 3C
· Above Aisle 7C

_____ 605.5 Extension Cords. Extension cords and flexible cords shall not be a substitute for permanent wiring. Extension cords and flexible cords shall not be affixed to structures, extended through walls, ceilings or floors, or under doors or floor coverings, nor shall such cords be subject to environmental damage or physical impact. Extension cords shall be used only with portable appliances:
· Under the make-up counter (manicure)

_____ 703.1 Maintenance. The required fire-resistance rating of fire-resistance-rated construction (including walls, fire stops, shaft enclosures, partitions and floors) shall be maintained. Such elements shall be properly repaired, restored or replaced when damaged, altered, breached or penetrated.
· Replace ceiling tile above aisle 7C

____ 901.6 Inspection, testing and maintenance. Fire detection, alarm and extinguishing systems shall be maintained in an operative condition at all times, and shall be replaced or repaired where defective. Nonrequired fire protection systems and equipment shall be inspected, tested and maintained or removed.
· Provide a sprinkler wrench in your "spare head" sprinkler box.
