07/18/2007 INSP - FULL Violation On 7-11-07, a fire prevention inspection was conducted a4801 East Broadway Blvd., in Tucson, Arizona.

The following have been referred to the building inspectors at City of Tucson Department of Neighborhood Services for possible building code requirements;

1). Separation and rated wall requirements between elevator area and office space.
2) Separation and rated wall requirements for exit, near former loading area on East Side first floor from bank.
3) Requirements to replace of fire resistive coating from I-beam support, in basement.

The following Tucson Fire Code Violation(s) was noted:

1). Fire drill is required. Drill date shall be determined by 9-5-07.
IFC 404.2. Fire drill shall be conducted with leadership, shall be unannounced, and accountability shall be performed. As per fire code requirements which state:

405.3 Leadership. Responsibility for the planning and conduct
of drills shall be assigned to competent persons designated
to exercise leadership.
405.4 Time. Drills shall be held at unexpected times and under
varying conditions to simulate the unusual conditions that occur
in case of fire.
405.5 Record keeping. Records shall be maintained of required
emergency evacuation drills and include the following
information:
1. Identity of the person conducting the drill.
2. Date and time of the drill.
3. Notification method used.
4. Staff members on duty and participating.
5. Number of occupants evacuated.
6. Special conditions simulated.
7. Problems encountered.
8. Weather conditions when occupants were evacuated.
9. Time required to accomplish complete evacuation.
405.6 Notification. Where required by the fire code official,
prior notification of emergency evacuation drills shall be given
to the fire code official.
405.7 Initiation. Where a fire alarm system is provided, emergency
evacuation drills shall be initiated by activating the fire
alarm system.
405.8 Accountability. As building occupants arrive at the assembly
point, efforts shall be made to determine if all occupants
have been successfully evacuated or have been accounted
for.
405.9 Recall and reentry. An electrically or mechanically operated
signal used to recall occupants after an evacuation shall
be separate and distinct from the signal used to initiate the evacuation.
The recall signal initiation means shall be manually operated
and under the control of the person in charge of the
premises or the official in charge of the incident. No one shall
reenter the premises until authorized to do so by the official in
charge.

2). Currently there are many holes in ceilings and wall in air handler rooms. All these holes shall be repaired with UL Listed products for the appropriate separation. IFC 703.1.1

703.1.1 Fireblocking and draftstopping. Required fireblocking and draftstopping in combustible concealed spaces shall be maintained to provide continuity and integrity
of the construction.

3). All combustible material shall be removed from all mechanical rooms. IFC 315.2.3
315.2.3 Equipment rooms. Combustible material shall not be stored in boiler rooms, mechanical rooms or electrical equipment rooms.

4). All parking area standpipe connections shall, be labeled for visibility when entering parking areas. IFC 912.4
912.4 Signs. A metal sign with raised letters at least 1 inch (25 mm) in size shall be mounted on all fire department connections serving fire sprinklers, standpipes or fire pump connections. Such signs shall read: AUTOMATIC SPRINKLERS or STANDPIPES or TEST CONNECTION or a combination thereof as applicable.

5). All valves to ALL standpipes shall be tested to ensure operation.
912.6 Inspection, testing and maintenance. All fire department connections shall be periodically inspected, tested and maintained in accordance with NFPA 25.

6). Fire sprinkler test and inspection reports shall be submitted and all required repairs shall be completed by 9-5-07. IFC 901.6
901.6 Inspection, testing and maintenance. Fire detection, alarm and extinguishing systems shall be maintained in an operative condition at all times, and shall be replaced or repaired where defective. Nonrequired fire protection systems and
equipment shall be inspected, tested and maintained or removed.




7). All material combustible are not that could affect or impede exiting in exit ways, exit passageways, and aisles shall be removed. This includes any items, which could be identified as trip or fall hazard while exiting. IFC 1027.2 and IFC 1027.3
1027.2 Reliability. Required exit accesses, exits or exit discharges shall be continuously maintained free from obstructions or impediments to full instant use in the case of fire or other emergency. Security devices affecting means of egress
shall be subject to approval of the fire code official.
1027.3 Obstructions. A means of egress shall be free from obstructions that would prevent its use, including the accumulation of snow and ice.

8). Currently many ceiling tiles are missing. These ceiling tiles are a required portion of the fire protection system and shall be replaced immediately after work is completed for the day or a break in the current task. IFC 703.1

703.1 Maintenance. The required fire-resistance rating of fire-resistance-rated construction (including walls, fire stops, shaft enclosures, partitions and floors) shall be maintained. Such elements shall be properly repaired, restored or replaced
when damaged, altered, breached or penetrated. Openings made therein for the passage of pipes, electrical conduit, wires, ducts, air transfer openings, and holes made for any reason shall be protected with approved methods capable of resisting the passage of smoke and fire. Openings through fire-resistance- rated assemblies shall be protected by self-closing o r automatic- closing doors of approved construction meeting the
fire protection requirements for the assembly.

9). Re install grid for rated ceiling tiles in 5th floor. IFC 703.1
10). Loading lock area. remove old fire alarm pull station. IFC 901.6
11). Replace ceiling door in old loading area. IFC 703.1
12). Elevator room. All combustible material shall not be placed within 30 inches of electrical panels. IFC 605.3
13). Remount portable fire extinguisher in suite 501. IFC 901.6
14). Gate in stairwell, shall be reinstalled to prevent exiting in stairwell.
Sign shall be posted on gate labeled; NOT AN EXIT As per IFC 1019.1.6 which states;
1019.1.6 Discharge identification. A stairway in an exit enclosure shall not continue below the level of exit discharge unless an approved barrier is provided at the level of
exit discharge to prevent persons from unintentionally continuing into levels below. Directional exit signs shall be provided as specified in Section 1011.
