07/12/2007 INSP - AST/UST Continued Removed the last three tanks. 2x12000 gallon and 1 x 16000 gallon. All tanks were inerted to below 5.9 % O2. Pictures were taken of the clevis that were welded on all three tanks without permission. Mike Kazz was told on previous meetings he was not to weld on the tanks as the MSDS on file for the product in the tanks forbids any welding.
07/12/2007 INSP - AST/UST Approved All tanks removed
07/10/2007 INSP - AST/UST Continued SI-- Piping being removed, 90% including the underground pipe has been removed.
06/15/2007 INSP - AST/UST Continued Four tanks were inerted to below 4% O2. These tanks were removed. 2x 12000 gallon, 1x 25000 gallon and 1x 16000 gallon. Mike Kazz is to contact Tim Murphy to coordinate the removal of the other three tanks. Piping should start to be removed next week.
06/14/2007 INSP - AST/UST Continued met with Mike Kazz, Tom Cooley, Ron Huxtable to discuss plans for removal on 6/15/07. They will need fire extinguishers, cylinders bonded to ASTs, approved labeling and remove piping. A safety meeting will be conducted prior to beginning the work tomorrow. They were informed that all personnel on site shall be in PPE. Area will be secured with fire line tape.
06/01/2007 INSP - AST/UST Continued Removal cancelled. The crane is unavailable. Tried to contact Mr Kazz and left a message with his office that a written contract must be submitted by 6-5-07. Mr. Kazz will not return my calls. Chief Uthe is in contact with him.
05/14/2007 INSP - AST/UST Continued Chief Uthe met with Mike Kazz. A plan was to be written and submitted for removel of the tanks. Chief Uthe will decide upon reciept of the plans.
04/26/2007 INSP - AST/UST Continued SI, Tanks are sealed properly and vapors were not detected with the monitor. Contacted the city real estate to see about an extension for the owner to remove the tanks safely.
04/24/2007 INSP - AST/UST Continued Met with Mike Kazz. Plan to remove 7 AST's 4-12000 2-16000 1-25000 Gallon. Conditions and methods that were to be used to purg the tanks were unsafe. Directed them to stop work. Tanks Shall be washed to a zero LEL before removal. And all waste disposed of correctly.
