Columbus Tucson, AZ. 85711 Mrs. Matinez The Tucson Fire Department has conducted a fire safety inspection at the above stated business on April 5, 2007. Below is a list of violations that shall be corrected to provide compliance with the applicable codes, regulations and ordinances. Compliance is required by the 2003 International Fire Code (with modifications). A Re-inspection will be performed on or about April 19, 2007 Notice of Violations: 605.4 Multiplug adapters. Multi-plug adaptors, such as cube adaptors, unfused plug strips or any other device not complying with the ICC Electrical Code shall be prohibited. · Remove all unapproved multi-plug adapters: - 2 Each in the room behind the dryers in the North laundry room 605.1/ 605.6 Unapproved conditions. Open junction boxes and open-wiring splices shall be prohibited. Approved covers shall be provided for all switch and electrical outlet boxes. · Replace the dead face panel in the room behind the North Laundry room · Replace the light switch cover in the maintenance shop · Replace the light switch cover in the room behind the South laundry room · Abate the exposed wires in the ceiling in the South Laundry room · Place a blank in the electrical box in the room behind the North Laundry room · Replace the missing electrical cover plates in the pool room · Abate the exposed wires on the light pole just East of the South East picnic area · Abate all electrical hazards in each picnic area: - Currently there are exposed wires coming out of the poles that are purposed to be outside lighting. (3 each locations) Hazardous Material Provide the following: 407.2 Material Safety Data Sheets. Material Safety Data Sheets (MSDS) for all hazardous materials shall be readily available on the premises. 407.4 Training. Persons responsible for the operation of areas in which hazardous materials are stored, dispensed, handled or used shall be familiar with the chemical nature of the materials and the appropriate mitigating actions necessary in the event of a fire, leak or spill. · Train employees on the location of the MSDS/ Hazardous Communication Program. 1008.1.8.3 Locks and latches. Locks and latches shall be permitted to prevent operation of doors where a readily visible durable sign is posted on the egress side on or adjacent to the door stating: THIS DOOR TO REMAIN UNLOCKED WHEN BUILDING IS OCCUPIED. The sign shall be in letters 1 inch (25mm) high on a contrasting background. · Place the approved sign above the main entrance doors in the Office 906.2 General requirements. Fire extinguishers shall be selected, installed and maintained in accordance with this section and NFPA 10. · Replace all missing fire extinguishers · South pool room extinguisher was last test in 1994 906.7 Hangers and brackets. Hand-held portable fire extinguishers, not housed in cabinets, shall be installed on the hangers or brackets supplied. · Properly mount the extinguisher in the lunch break room in the office. 907.2.10.1.2 Groups R-2, R-3, R-4 and I-1. Single or multiple-station smoke alarms shall be installed and maintained in Groups R-2, R-3, R-4 and I-1 regardless of occupant load at all of the following locations: 1. On the ceiling or wall outside of each separate sleeping area in the immediate vicinity of bedrooms. 2. In each room used for sleeping purposes. - Provide working smoke detectors for the following units: - #6, #375, #120, #307 - Provide written record that all smoke detectors have been checked by your personnel. 110.1.1 Unsafe conditions. Structures or existing equipment that are or hereafter become unsafe or deficient because of inadequate means of egress or which constitute a fire hazard, or are otherwise dangerous to human life or the public welfare, or which involve illegal or improper occupancy or inadequate maintenance, shall be deemed an unsafe condition. A vacant structure which is not secured against unauthorized entry as required by Section 311 shall be deemed unsafe. · Currently the room where you store and charge your golf cart has carpet on the floor that is saturated with oil and other combustible liquids. This hazard shall be immediately abated. · Your pool gates have numerous opening that are larger than 4 inches. This is a distinct danger to human life and welfare. You are hereby ordered to abate this immediately. - East side of the north pool has openings near the ground larger that 4 inches. - East side of the north pool has openings where the th erod iron has been removed. The opening is Approximately 12 inches by 24''. · All pool gates shall operate correctly. The Tucson Fire Department is committed to the protection of your property.
