Location has a valid license to operate an adult care facility for developmently disabled posted from the AZ Department of Health Services. A certificate of occupancy was not posted at the facility. The site manager, Dorothy, stated one was on file in the Regional Director's office, but she was out of the office. Dorothy stated a copy will be faxed over to DNR Monday. Dorothy also advised the site was operating a day time activity program a few weeks ago for under 10 people and that they also had a State issued license for that program. They are not currently running day time activities and do not foresee any in the recent future. Note: Apparently there was an incident with one of the residents involving one of the neighbors recently, which likely prompted the call to DNR and the Ward Office. Both types of activities are allowable in this residential area (R1). Should the management not provide a certificate of occupancy, a 30-day notice of violation will be issued to obtain one. LRP
