10/08/2007 INSP - FOLLOW UP CODE ENFORCE Approved INSPECTOR MALDONADO AND MYSELF HAD AN INSPECTION WITH THE BUSINESS OWNER. THE BUSINESS OWNER IS READY TO DO ALL THAT IS REQUIRED TO GET BACK IN BUSINESS AND BELOW IS THE SUMMARY OF THE REQUIREMENTS.

A) COMPLETELY REMOVE AND DISCARD SPRAY BOOTH.

B) REMOVE ALL OF THE WIRING THAT IS VISABLE, GOING TO THE SPRAY BOOTH, AUTO REPAIR AREA, COMPRESSOR AREA AND THE EXPOSED UNPERMITTED ITEMS THAT EXIT THE SERVICE PANEL BOARD.
A FUTURE RECONNECTION WILL REQUIRED PERMITS.

C) PROPERLY REGROUND THE SERVICE. LOOSE GROUNDING CONDUCTORS. NEED K.O. SEALS AND FILLER PLATES. ALL WORK MUST BE BY A LICENSED C-11 ELECTRICAL CONTRACTOR.

D) HAVE THE ELECTRICAL CONTRACTOR CHECK ALL OF THE ELECTRICAL AND ALSO MAKE CORRECTION MAY BE NEEDED.

E) THE BATHROOM REQUIRES A GFCI RECEPTICLE.

F) NO STORAGE ALLOWED IN OFFICE AND AT PROPERTY. TOOLS AND MINOR ITEMS O.K.

G) THE LAVATORY IS FALLING AND NEEDS WORK.

H) POWER WILL NOT BE RESTORED UNTIL A FINAL OR IF CONTRACTOR SCHEDULES WITH THE DNR INSPECTOR.

I) THE ELECTRICAL CONTRACTOR MUST SUBMIT IN COMPANY LETTERHEAD ON INVOICE OR LETTER STATING THE WORK DONE AND INDICATING THAT HE /SHE DID WORK.
10/05/2007 INSP - FOLLOW UP CODE ENFORCE Approved 12:30 PM ARRIVED AT SITE AND INSPECTOR MALDONADO WENT TO TALK WITH THE OWNER MR. LEE. WHO DID NOT WANT TO GO TO THE SITE. JESSE TOLD HIM OF IMPENDING "SHUT DOWN"

12:40 PM POSTED PROPERTY WITH "DO NOT ENTER" FORMS AT OFFICE DOOR, SOUTH PART OF OFFICE AND AT THE SPRAY BOOTH.

SPOKE WITH THE TENANT THAT THE SITUATION IS GRAVE AND DANGEROUS AND THAT NUMBEROUS TIMES HE HAS DEFIED DNR'S ORDER NOT TO USE THE SPRAY BOOTH AND AUTO REPAIR FACILITIES.

WE TOLD THE TENNAT THAT WE WILL BE BACK IN ONE HOUR, THAT HE MUST GATHER AL OF HIS PERSONAL BELONGINGS BECAUSE NO ENTRY WILL BE ALLOWED AND THE POWER WILL BE SHUT OFF WITH NOTIFICATION TO T.P.D. TO ARREST ANYONE AT SITE UNLESS AUTHORIZED BY DNR.

INSPECTOR MALDONADO AND MYSELF ALSO EXPLAINED THAT WITH THE APPROVAL FROM DSD IT MAY BE POSSIBLE TO OBTAIN A TEMP C OF O FOR THE SALES OFFICE ONLY, BUT STILL AN INSPECTION APPROVAL FOR A C OF O WILL BE REQUIRED.

ARRIVED AT SITE AND THE MAIN STRUCTURE (OFFICE) WERE ALL LOCKED AND SECURED. NO ONE WAS THERE. MR. MALDONADO CALLED THE TENANT TO LET HIM KNOW THAT WE ARE SHUTTING THE POWER AND PAD LOCKING THE SERVICE EQUIPMENT "OFF" AND PAD LOCKING THE SPRAY BOOTH. THE TENANT SAID O.K.

WE POSTED THE 'NOTICE TO VACATE' AND TOOK PHOTOS.

3:54 PM CALLED T.P.D. NOTIFIED DISPATCH OF CASE. NOT TO LET ANYONE ENTER WITHOUT AUTHORIZATION FROM D.N.R.

THE POWER WAS DISCONNECTED FOR HAZARDOUS CONDITIONS.

NOTIFIED POLICE ON "BEAT" TO ARREST ANYONE AT THE PREMISES.

10/04/2007 INSP - FOLLOW UP CODE ENFORCE Violation Inspector found spray paint both open with auto parts (fenders,hood) being painted. Previous postings where removed from entry and electrical connections thruough the property. Paint on fender and hood was fresh. Brothers owner had denied the fact about him painting the auto parts.
07/05/2007 INSP - FOLLOW UP CODE ENFORCE Correction Required A FOLLOW-UP INSPECTION WAS MADE WITH Jessee Maldonado and myself. Nothing to remedy the situation has taken place and as a matter of fact the situation has regressed. I called Blizzard Elerctric to see what happened with his contract to make the corrections and he stated that he was never called back.
01/03/2007 INSP - FOLLOW UP CODE ENFORCE Approved INSPECTOR MOLDANADO AND MYSELF MADE AN inspection of the facility and found that the spray booth was erroneously rewired and the NOV and the UNSAFE TO OCCUPY posting was also removed. Jesus and myself reitterated to the owner that it is required to first get a permit along with plans, not to remove the POSTING or other penalties would occure. An electrical engineer is required for any plans in a classified location. See photos (this date) in SIRE.
