02/15/2006 INSP - FULL Violation On 12-9-05 and 12-12-05, a fire prevention inspection was conducted at 9280 East Old Vail Road, in Tucson, Arizona.

The reinspection was conducted on 2-7-06, at the above address.
In attendance were Captain Ron Summers, Captain Don Uthe and the crew of Engine Company 19.

The attached letter is your fire inspection report.

The following International Fire Code Violation(s) was noted:

1). All spray booths using flammable finishes shall be approved by City of Tucson Development Services at 201 North Stone, 791-5550.
A permit is required to operate these spray booths as per IFC 1501.2.
Spray booths shall be removed from building if approval is not granted.

If approval for all spray booths is not obtained by City of Tucson, or spray booths are not removed by 3-30-06, these violations shall be sent to City of Tucson City Prosecutors Office for review and possible litigation.

2). All containers storing flammable liquids shall be in accordance with IFC 3404.3.1, which states:

3404.3.1 Design, construction and capacity of containers and portable tanks.
The design, construction and capacity of containers for the storage of flammable and combustible liquids shall be in accordance with this section and Section 4.2 of NFPA 30.

3404.3.1.1 Approved containers.
Only approved containers and portable tanks shall be used.

The Tucson Fire Department does not approve flammable liquid container storage without secure lids. All containers without secure lids shall be photographed and presented to City of Tucson City Prosecutors Office for review and possible litigation.

3). 1504.2 Ventilation.
Mechanical ventilation of spraying areas shall be provided in accordance with Section 510 of the International Mechanical Code.

1504.2.1 Operation of spraying areas.
Mechanical ventilation shall be kept in operation at all times while spraying operations are being conducted and for a sufficient time thereafter to allow vapors from drying coated articles and finishing material residue to be exhausted. Spraying equipment shall be interlocked with the ventilation of the spraying area such that spraying operations cannot be conducted unless the ventilation system is in operation.

City of Tucson mechanical inspector shall determine make up air requirements in all spraying areas.

4). Enclosed processing areas using flammable liquids, shall be ventilated as per NFPA 30 section 7.3.4.1.
As of 3-30-06 ventilation shall be provided to these areas.
Failure to provide adequate ventilation in these areas shall result in the following steps;
A photograph of processing area, and work description shall be presented to City of Tucson City Prosecutors Office for review and possible litigation.

In addition the following fire code violations were noticed during the reinspection on 2-7-06.

5). Wood waste was allowed to accumulate around machines, and numerous machines did not have dust collection.
This is in violation of:
1903.2 Dust control.
Equipment or machinery located inside buildings which generates or emits combustible dust shall be provided with an approved dust collection and exhaust system installed in accordance with Chapter 13 and the International Mechanical Code. Equipment or systems that are
used to collect, process or convey combustible dusts shall be provided with an approved explosion control system.
1903.3 Waste removal.
Sawmills, planning mills and other woodworking plants shall be equipped with a waste removal system that will collect and remove sawdust and shavings. Such systems shall be installed in accordance with Chapter 13 and the International Mechanical Code.
Exception: Manual waste removal when approved.
Manual waste removal must be enforced, and only as a secondary means for housekeeping.
1903.3.1 Housekeeping.
Provisions shall be made for a systematic and thorough cleaning of the entire plant at sufficient intervals to prevent the accumulations of combustible dust and spilled combustible or flammable liquids.
Cleaning intervals must be increased to prevent this accumulation.

6) Unsecured LPG bottle in NE corner of yard, shall be secured. IFC 3003.3.2
7). All tanks with hazardous materials in stationary containers and above ground tanks shall be marked and labeled. As per 2703.5 which states;
2703.5 Hazard identification signs.
Unless otherwise exempted by the fire code official, visible hazard identification signs as specified in NFPA 704 for the specific material contained shall be placed on stationary containers and above-ground tanks and at entrances to locations where hazardous materials are stored, dispensed, used or handled in quantities requiring a permit and at specific entrances and locations designated by the fire code official.
2703.5.1 Markings.
Individual containers, cartons or packages shall be conspicuously marked or labeled in an approved manner. Rooms or cabinets containing compressed gases shall be conspicuously labeled: COMPRESSED GAS.
2703.6 Signs.
Signs and markings required by Sections 2703.5 and 2703.5.1 shall not be obscured or removed, shall be in English as a primary language or in symbols allowed by this code, shall be durable, and the size, color and lettering shall be approved.
Currently many containers are not marked.

8). Exit sign out of service in plating area shall be repaired. IFC 1026.3
9). Portable fire extinguisher blocked by flammable liquid storage and shall be moved. IFC 906.6
10). Automatic fire sprinkler test and inspection report shall be completed and all repairs shall be completed by 3-30-06. IFC 901.6
11). All wiring from sprinkler piping shall be removed, and all dust shall be removed from sprinkler heads. 901.6 BY 3-30-06
12). All electrical service equipment shall be in accordance with IFC 605.3 which states;
605.3 Working space and clearance.
A working space of not less than 30 inches (762 mm) in width, 36 inches (914 mm) in depth and 78 inches (1981 mm) in height shall be provided in front of electrical service equipment. Where the electrical service equipment is wider than 30 inches (762 mm), the working space shall not be less than the width of the equipment. No storage of any materials shall be located within the designated working space.
Exceptions:
1. Where other dimensions are required or allowed by the ICC Electrical Code.
2. Access openings into attics or under-floor areas which provide a minimum clear opening of 22 inches (559 mm) by 30 inches (762 mm).

Furniture, work benches and storage must be moved.

13). Plating dept. office. Repair electrical on outside. IFC 605.3
14). Remove obstruction to sprinkler head. IFC 901.6
15). All hazardous material spills (liquids and sludge)shall be PROMPTLY cleaned and made harmless and disposed of promptly and correctly.
IFC 2701.3.3.2

16) A Hazardous Material Inventory Statement shall be submitted to The Tucson Fire Department. This shall include ALL the following information. IFC 2701.5.2
2701.5.2 Hazardous Materials Inventory Statement (HMIS).
Where required by the fire code official, an application for a permit shall include an HMIS, such as SARA (Superfund Amendments and Reauthorization Act of 1986) Title III, Tier II Report, or other approved statement. The HMIS shall include the following information:
1. Manufacturer's name.
2. Chemical name, trade names, hazardous ingredients.
3. Hazard classification.
4. MSDS or equivalent.
5. United Nations (UN), North America (NA) or the Chemical Abstract Service (CAS) identification number.
6. Maximum quantity stored or used on-site at one time.
7. Storage conditions related to the storage type, temperature and pressure.


17). IFC 340.3.1. Where flammable liquids are used, provisions shall be shown to The Tucson Fire Department for prompt and safe disposal of leakage or spills. This shall include kits and absorbent material. NPFA 30 (7.3.7.2)

Also;
Update emergency numbers in lock box, and as always, all permits must be posted in during new construction.

This letter is to inform you that abatement shall be March 30, 2006, on all the above violations.
A re-inspection shall be conducted at that time to ensure compliance.
