10/31/2005 | INSP - REINSPECTION | Violation | On 10-28-05, a fire warden meeting was conducted at 1 South Church. The following International Fire Code Violation(s) was noted at the meeting: 1). The occupants are concerned that the fire alarm could not be heard on the following areas on the building. They are: 15th floor, 16th, 10th, 9th, 10th, Fire alarms shall be of the following decibels as per IFC 907.10.2 Which states: 907.10.2 Audible alarms. Audible alarm notification appliances shall be provided and sound a distinctive sound that is not to be used for any purpose other than that of a fire alarm. The audible alarm notification appliances shall provide a sound pressure level of 15 decibels (dBA) above the average ambient sound level or 5 dBA above the maximum sound level having a duration of at least 60 seconds, whichever is greater, in every occupied space within the building. The minimum sound pressure levels shall be….. 60 dBA in other occupancies The decibels of fire alarm in offices shall be 60 dBA minimum in all offices or 15 dBA average ambient sounds. Within 14 days an appointment will be made to check the sound levels in theses area. Reinspection may be held after normal business hours. In addition; no strobes are on the 14th floor, 15th, 16th, and 17th. 2) A reviewing the fire evacuation plan, all references to calling management office DURING an emergency shall be removed, and new plan shall be included. The NEW evacuation plan shall have the following included. 404.3.1 Fire evacuation plans. Fire evacuation plans shall Include the following: 1. Emergency egress or escape routes and whether evacuation of the building is to be complete or, where approved, by selected floors or areas only. 2. Procedures for employees who must remain to operate critical equipment before evacuating. 3. Procedures for accounting for employees and occupants after evacuation has been completed. 4. Identification and assignment of personnel responsible for rescue or emergency medical aid. 5. The preferred and any alternative means of notifying occupants of a fire or emergency. 6. The preferred and any alternative means of reporting fires and other emergencies to the fire department or designated emergency response organization. 7. Identification and assignment of personnel who can be contacted for further information or explanation of duties under the plan. 8. A description of the emergency voice/alarm communication system alert tone and preprogrammed voice messages, where provided. 404.3.2 Fire safety plans. Fire safety plans shall include the following: 1. The procedure for reporting a fire or other emergency. 2. The life safety strategy and procedures for notifying, relocating, or evacuating occupants. 3. Site plans indicating the following: 3.1. The occupancy assembly point. 3.2. The locations of fire hydrants. 3.3. The normal routes of fire department vehicle access. 4. Floor plans identifying the locations of the following: 4.1. Exits. 4.2. Primary evacuation routes. 4.3. Secondary evacuation routes. 4.4. Accessible egress routes. 4.5. Areas of refuge. 4.6. Manual fire alarm boxes. 4.7. Portable fire extinguishers. 4.8. Occupant-use hose stations. 4.9. Fire alarm annunciators and controls. 5. Alist of major fire hazards associated with the normal use and occupancy of the premises, including maintenance and housekeeping procedures. 6. Identification and assignment of personnel responsible for maintenance of systems and equipment installed to prevent or control fires. 7. Identification and assignment of personnel responsible for maintenance, housekeeping and controlling fuel hazard sources. All list of fire wardens shall be reestablished and submitted to TFD, and a system for accountability shall be re-established. AS per IFC 405.8 which states; 405.8 Accountability. As building occupants arrive at the assembly point, efforts shall be made to determine if all occupants have been successfully evacuated or have been accounted for. 3). A letter shall be sent to confirm all fire alarm testing is complete, from the annual fire inspection. This letter is to inform you that abatement of these International Fire Code violations must be completed by 11-14-05. A re-inspection shall be conducted at that time to ensure compliance. |
