01/22/2008 GRADING - FINAL Continued Contractor called in final inspection, contractor still has not provided revised drawings. I told Wolf site super I will hold inspection for 10 days while he tries to obtain drawings. after 10 days if he does not have drawings inspection will be denied.
01/10/2008 GRADING - FINAL Continued Met with Wolf, Jason Green, and Eng. at site today to review Basins, storm outlet at NW corner, and inlets to storm pipes. It was decided that the rip rap at oufall to pipe at NW corner will be lowered approx 1ft to allow water to flow to the NW. The depressions in front of the inlets pipe need to be filled. eng will determine how. Basin will be scarified before seeding for better perc. rate.
12/04/2007 GRADING - FINAL Denied Contractor needs to address preliminary punch list:
1. Provide letter from engineer in regards to drainage, per chapter XIV, section 14.2 (7).
2. Clean all streets and parking areas.
3. Stabilize all areas disturbed by construction. (on and off site)
4. Install rip rap at all curb cuts. (DG washing out in to roadway)
5. Complete all landscaping.
6. Install splash pad at all roof drains.
7. Complete all basin work per approved plan. Have all changes approved at 201 N. Stone Ave.
8. Engineer needs to address basin discharge. (erosion to neighbors property)
9. Separate all construction activity from public access.
10. Remove port-a-pottys, dumpster and mini mobile storage units from site. (Contractor can obtain permit for mini mobile units. Must have permit before final inspection.)
11. Complete all sidewalk work.
12. Remove construction debris from site, drainage channels and from around perimeter wall.
13. Paint bollards in trash enclosure.
14. Address ponding water in parking area. (northeast corner)
15. Complete all parking lot striping and signage installation.
16. Install safety barrier for detention basin.
17. Install bike parking.
18. Clean all wash out from around site.
19. Engineer needs to address ponding water at southeast inlet. (6+")
11/14/2007 GRADING - FINAL Correction Required Issued correction notice. Contractor has 30 days to address:
1- Stabilize all areas disturbed outside the permeter wall.

11/14/2007 GRADING - FINAL Partial Approval Met Kevin on site. Looked at sidewalk eastside that goes from right-of-way to building. Contractor to install flat area per ADA.
11/08/2007 GRADING - FINAL Partial Approval Inspection headwall on southwest corner. Re-bar install per approved detail. Spoke to Kevin in regards to stabilizing outside perimeter wall. All areas disturbed by construction.
11/01/2007 GRADING - FINAL Not Applicable Met with Wolf on site, contractor needs to stretch out H.C. ramp on southwest corner to met slope requirement.
10/31/2007 GRADING - FINAL Not Applicable Spoke to Wolf in regards to handrail on ramp west side of building. Ramp will need handrail on both sides per ADA 4.8.5. Rise greater then 6" and a projection more then 72". Will met with Wolf on site.
10/26/2007 GRADING - FINAL Not Applicable Met with Wolf and Bill on site. Went over HC ramps at entrance, made minor changes to ramp. Also contractor will needs to contact Jason Green in regards to changes to basin side slopes.
10/23/2007 GRADING - FINAL Not Applicable Met on site with Wolf site super and Brian site engineer. Contractor needs to install access ramp in to basin. If contractor installs ramp on northeast corner will be able to maintain 15% slope.
10/11/2007 GRADING - FINAL Not Applicable Met Wolf on site went over minor changes to H.C. ramps. (headers and truncated domes)
10/04/2007 GRADING - FINAL Not ready for Inspection Met with Wolf on site, to go over site and grading plans. Wolf will need address:
1- Slope bank along north and west perimeter wall.
2- Slope bank in detention basin.
3- Stabilize all areas disturbed.
4- Clean existing drainage channel south side of project.
5- Address sidewalk slope west side on building (across from Basin) HC parking may need to be moved.
Also spoke to Wolf about standing water in asphalt.
06/26/2007 GRADING - FINAL Partial Approval Contractor placed rebar for fotter and wing wall on southwest drainage inlet, and footer for discharge weir. Rebar for both placed per approved details.
05/30/2007 GRADING - FINAL Partial Approval Head wall inspection at NW corner of site, approved to pour concrete
05/29/2007 GRADING - FINAL Partial Approval Contractor placed rebar for head wall on east side inlet, per detail ADOT B-11.12.
05/17/2007 GRADING - FINAL Partial Approval Contractor installed headwall on northwest corner of project per ADOT B11.12. (next to basin)
05/10/2007 GRADING - FINAL Partial Approval Contractor formed bases for manholes 2 and 4. Contractor placed concrete for walls on manholes 1 and 3. Per standard details.
04/30/2007 GRADING - FINAL Partial Approval Stormwater manhole #1 and 2 built per COT standard detail.
04/20/2007 GRADING - FINAL Partial Approval Contractor continues to place sand around 66" pipe.
04/19/2007 GRADING - FINAL Partial Approval Met with Wolf site super and engineer on site, went over installation of storm pipe and the need for compaction test. Contractor installed approx. 350' of 66" pipe.
03/09/2007 GRADING - FINAL Correction Required Contractor placed trailer off site. Contractor will need to get permision from owner to use area or move trailer. Extra SWPPP controls will need to be installed around trailer. Spoke to "Wolf" site super in regards to issue.
