POWER WAS CUT AT THE MIDSPAN ON 9/23/03 DUE TO UNAUTHORIZED USAGE. METER # MC14976 SHOWS NO ELECTRICAL SERVICE AT THIS TIME. READ OFF THE OTHER 3 METERS ON PROPERTY TO NORMA. UNITS #1,2,3&4 ON PROPERTY. UNIT #3 ONLY ONE WITHOUT POWER. GAS METER ALSO MISSING TO UNIT #3. TENANT STATES HE HAS NEVER HAD A GAS METER. METER HAS BEEN REMOVED BY SOUTHWEST GAS. CONTACTED RICK S. FOR SHUT DOWN APPROVAL. CONTACTED JUDY KIDNEY FOR POSSIBLE PLACEMENT OF TENANT. COMPLETED INSPECTION OF INTERIOR OF UNIT. FOUND: -NO CENTRAL HEAT. -EXTERIOR DOORS (3) 1 SCREWED SHUT AT KITCHEN SOUTH WALL. REAR KITCHEN DOOR IN WEST WALL IN NEED OF REPLACEMENT. DOOR IS DAMAGED BEYOND REPAIR. FRONT EXTERIOR DOOR IN LIVING ROOM EAST WALL LACKS WEATHER STRIP , SWEEP AND THRESHOLD. DOOR KNOB DOES NOT FUNCTION PROPERLY. -COOLER IN KITCHEN WINDOW HAS EXPOSED SPLICED WIRING TO PUMP. A/C WINDOW UNIT IN BEDROOM BLOCKS EGRESS FOR EMERGENCY ESCAPE OR RESCUE. -EXPOSED WIRING AT REAR DOOR JAMB AND CLOSET IN KITCHEN WEST WALL. HAZARDOUS ELECTRICAL. -NO GAS UTILITIES. NO GAS METER. -ROOF LEAK AT ADDED EXHAUST FAN IN LIVING ROOM. AT VENT PENETRATION IN CEILING. -NO HOT WATER TO ANY WATER FIXTURES. -SHOWER AREA WINDOW DOES NOT OPERATE PROPERLY FOR VENTILATION. -SHOWER FIXTURES LEAK. -TENANT IS HANDICAPPED AND DWELLING IS NOT HANDICAPP ACCESSIBLE. RESERVED ROOM AT ROADWAY INN FOR TENANT THROUGH JUDY KIDNEY. ONE SMOKING ROOM ON BOTTOM FLOOR FOR WHEELCHAIR ACCESSIBILITY. ATTEMPTED TO CONTACT TENANTS CASE MANAGER (LEILAH 546-4445) WHILE ON SITE. LEFT SEVERAL VOICE MESSAGES BUT NEVER RECEIVED ANY CALLS BACK. CASE MANAGER IS WITH PIMA HEALTH SYSTEMS. CONTACTED TENANTS NURSE (MORGAN 406-5370) WITH DEPENDABLE NURSING. SHE WAS SCHEDULED TO ARRIVE AROUND 5:30 PM TONIGHT. NURSE DID ARRIVE A LITTLE AFTER 5:30 P.M. SHE STATED THE POWER HAS BEEN OFF FOR MONTHS. SHE WORKS WITH TENANT 2 HOURS A DAY FROM 5-7 PM. SHE HAS BEEN CONCERNED OF THE LIVING CONDITIONS OF THE TENANT. SHE HAS SPOKEN TO THE OWNER OF THESE CONCERNS. NURSE WAS VERY HELPFUL THROUGHOUT SHUT DOWN WITH TENANT. A COUPLE OF TENANTS FRIENDS ALSO SHOW GREAT CONCERN FOR HIS LIVING CONDITIONS. AT THIS TIME DWELLING HAS BEEN DEEMED UNSAFE TO OCCUPY BECAUSE OF LACK OF 2 REQUIRED UTILITIES. ELECTRICAL AND GAS UTILITIES. INSPECTOR FEELS THIS IS CAUSING UNSANITARY LIVING CONDITIONS TO THE TENANT. NO CENTRAL HEAT. NO HOT WATER TO ANY WATER FIXTURES. NO LIGHT AT NIGHT FORCING TENANT TO USE PROPANE GAS LAMPS TO LIGHT THE DWELLING. DURAFLAME FIREPLACE LOGS FOUND ON SITE UNDER TENANTS BED. JUDY KIDNEY NOT ABLE TO GET A BUS PASS FOR TENANT TO TRAVEL TO RODEWAY INN. TENANT, FRIENDS ON SITE OR NURSE DO NOT HAVE TRANSPORTATION TO TRANSPORT TENANT. SEVERAL ATTEMPTS TO CONTACT AGENCIES TO TRANSPORT TENANT HAVE FAILED. NO ONE ABLE TO ARRIVE UNTIL THE NEXT MORNING. CONTACTED RICK S. WITH PROBLEM OF TRANSPORTATION OF TENANT. RICK CONTACTED VICTIM WITNESS THROUGH T.P.D. FOR POSSIBLE ASSISTANCE FOR TRANSPORTATION. RICK WAS ABLE TO GET TRANSPORTATION TO TENANT FROM VICTIM WITNESS. LARRY FROM VICTIM WITNESS ARRIVED ON SITE WITH A MINI-VAN WHICH WAS NOT WHEELCHAIR ACCESSIBLE. TENANT IS IN AN ELECTRIC WHEELCHAIR WHICH IS VERY HEAVY AND WOULD BE IMPOSSIBLE TO GET HIM IN MINI-VAN. SEVERAL ATTEMTS AGAIN TO CONTACT DIFFERENT AGENCIES TO TRANSPORT TENANT HAVE FAILED. LARRY FROM VICTIM WITNESS THEN CONTACTED HANDI-CAR WHO COULD TRANSPORT TENANT AND WOULD ARRIVE IN ABOUT AN HOUR. TENANT HAS SECURED DWELLING. POSTING DWELLLING HAS BEEN DEEMED UNSAFE TO OCCUPY ON FRONT DOOR. SEE PICTURES. SEND NOTICE. DISCUSSED CASE WITH RICK S. ON PHONE POSSIBLE SABER CASE. |
