FLOOR HAS BEEN DAMAGED, CRACKED & HAS BEGUN TO WARP. NO OTHER VIOLATIONS WERE FOUND. SMOKE DETECTOR WAS TESTED & WORKS FINE. WILL ISSUE NOV & ALLOW 30 DAYS TO ABATE.
